Rapid Response Team- Croydon Community Health Services.
Providing rapid assessment and appropriate rehabilitation to clients in order to prevent hospital admission and provide early supported discharge in a rapid manner.
To manage a caseload of clients with complex needs, using evidence based/client centered principles to assess, plan, implement and evaluate interventions in their own home, or in a rehabilitation bed.
To provide leadership for junior staff through supervision, competencies and appraisal.
To ensure that intervention is responsive, and sensitive to the social, cultural, linguistic and religious needs of clients in accordance with equal opportunity policies.
To act as clinical lead in place of the Operational Clinical Lead’s absence/delegation.

To provide Occupational therapy assessment and intervention within an acute winter pressures ward. Croydon University Hospital has an increased number of adult and elderly admissions during the winter months, and therefore required additional Therapists and Medical staff to deal with this increased number of inpatients.

Reablement and Case Manager, The community Independence Service, Hammersmith & Fulham
Therapy lead role assessing physically disabled people within the borough, who are experiencing functional difficulties at home, or who have recently been discharged from hospital from hospital with increased levels of support, with an aim at increasing independence through rehabilitation, or using compensation techniques via equipment, adaptations, or accessing other services.
Duty senior role also included in this post where I would screen, accept or reject inappropriate services, including sign posting to more relevant services within the borough that meet an individual’s needs more appropriately.
Supervision and training to unqualified rehabilitation support workers.

Croydon University Hospital, A&E Liaison (community based service within A&E)
To carry out OT assessments of patients referred by A&E and work with the MDT to assess suitability to return home
To provide follow-up intervention within the patient’s home, to ensure the patient remains safe and independent
To liaise with other community services involved in supporting vulnerable adults to remain at home in order to support living and prevent emergency admission into hospital

- Recommendations of specialist equipment, Housing needs reports, and minor and major adaptations, as well as advice for non-eligible OT needs

- Responsible for up to 30 elderly rehab patients, including up to 10 patients with slow stream orthopedic needs
