Job Title: / Physiotherapist – Band 6
Salary: / £26,041 - £34,876 per annum pro rata
Reports to: / Rapid Response Team Leader
Job Purpose: / The post holder will work as a key member of the Rapid Response Team assessing patients in order to prevent hospital admission and working alongside the In Reach Team prioritising patients who are appropriate for early discharge from hospital.
Key Responsibilities: /
  • The post holder will undertake a holistic assessment of a variety of disorders, diagnose and plan the rehabilitation as part of a multi-disciplinary service approach in order to provide a co-ordinated package of care and safe discharge from the service.
  • Assess, prescribe, provide and order appropriate equipment within a set budget or to refer on as necessary (e.g. to Care and Repair or Social Services for ramps, rails, adaptations etc).
  • Carry out skilled physiotherapy intervention drawing upon wide ranging evidence based and problem solving professional knowledge and skills, gained through specific training and practical experience.
  • Actively promote inter-disciplinary collaboration by working closely with other team members and by having a thorough understanding of their roles and the role of the Urgent Care
  • Supervise junior staff members, students and rehabilitation assistants carrying out delegated physiotherapy exercise programmes with service users.
  • Contribute to the in house training programme by undertaking relevant teaching sessions as well as support of junior staff with their competency programmes.
  • Adapt treatment programmes to suit the patients home environment.
  • Understand the constraints and flow of patients through the maximum 7 day remit of the Rapid Response Team.
  • Work as an autonomous community practitioner
  • To provide support for the Lead Physiotherapist and ANP to ensure the smooth and efficient running of Early Discharge Liaison service
  • Maintain and complete appropriate and timely patient clinical records in line with professional and NHS Trust guidelines

Skills specification: /
  • A high level of effective communication and an ability to develop relationships with patients, carers and hospital staff.
  • Competent IT skills, use of core Microsoft Office packages, ability to generate clinical and statistical reports
  • The ability to work on own initiative, make decisions and manage risk in a variety of clinical and home environments
  • A good degree of prioritisation skills and the ability to manage, organise and delegate own caseload.
  • Demonstrable people management skills, you will need to be able to supervise and manage and support decision making with junior staff and students.

Experience: /
  • Evidence of completion of the core Physiotherapy rotations including neurological, orthopaedic, respiratory and elderly care
  • Documented evidence of CPD in accordance with HPC recommendations
  • Previous experience in working with elderly patients within the field of Urgent Care/Rehabilitation
  • Experience in working in a Hospital/ community /intermediate care setting.

Knowledge and qualifications: /
  • Diploma/ degree in physiotherapy
  • HCPC registration
  • Have a broad knowledge of Adult Community Care
  • Member of the chartered society of physiotherapy
  • To be able to travel between a variety of locations within the Trust. A motorised vehicle is required
