Service.

That the company shall not employ foreign national beyond the period authorized by the Department of Justice.

That it shall likewise assume responsibility for the voluntary departure of said foreign national and dependents upon termination of employment/training and defray all expenses thereof.

That it shall train at least two (2) Filipinos in the financial / administrative and/or supervisory skills for which employment of the foreign national is required.

That it shall cause the registration of the foreign national with the Board of Investments within sixty (60) days from his arrival or within thirty (30) days after commencement of employment of the foreign national is required.

That it shall submit to the Board of Investments of the Understudy Training Program within thirty (30) days from issuance of the visa.

II.Documents to be submitted:

1)For supervisors, specialist, consultants, contractors, personal staff and dependents;

a)Valid passport/s of foreign national and his/her dependents, if any subject to the application, and when required by the Department of Justice, his/her their re-entry permit/s to port of embarkation or country of origin;

b)Certificate of registration issued by appropriate agency, of the employer/sponsor, if engaged in the business (BOI Cert. of Reg’n.);

c)Copy of the contract/agreement entered into by the employer/sponsor of the prospective special non-immigrant, with a government office, agency; Secretary’s Certificate in case the prospective non-immigrant is a elected officer:

d)Proof of relationship of dependents, such as marriage certificate of spouse and birth certificate of children and affidavit of support;

e)BOI endorsement letter/approval.

2)For exchange of professors, fellows, students, scholars, participants, volunteers and social workers under sponsorship of locally or internationally recognized educational, scientific, cultural, relief and charitable organizations, institution or agencies or foundations and representative of non-recognized foreign government to international organization and their dependents;

a)Valid passport of foreign national, subject of the application, and when required, re-entry permit to point of embarkation or country of origin.

b)Contract or agreement between local sponsors and foreign national, if any

c)Appointment of foreign national by the host or receiving organization or, institution or foundation.

d)Acceptance signed by the subject foreign national.

e)Affidavit of support and guarantee of return fare by the sponsor.

f)Proof of guarantor’s financial capacity to fulfill his/her undertaking, e.g. income tax returns, bank accounts, etc.
