JOB DESCRIPTION

POST:Family Well-being Practitioner (LCC)

SALARY:Scale range 21 – 25 (£19,941to £22,437) pa pro rata

HOURS:22.5 hours per week

(flexible working is required and the post holder will be required

to work evenings and weekends, as agreed to meet the needs of the Service)

LOCATION:Based at Preston New Road office working across Lancashire and other locations as required

ANNUAL LEAVE25 days plus Bank Holidays pro rata

(rising to 30 days after five years’ service)

1PRIMARY PURPOSE OF THE POST

1.1To be responsible for an allocated locality within the geographical area of Lancashire and to be a designated ‘Key Worker’ for the locality / cluster group.

1.2To deliver parenting interventions to support parents of children and young people involved in the Familywise service to work towards positive change. The Service will develop a comprehensive systemic response using models of evidence-based practice.

1.3 To work in partnership with others, to support and enable children, young people and their families to achieve emotional health and physical well being.

1.4Liaise and work in partnership with schools and local agencies to co-ordinate appropriate, timely family support services and to promote a holistic ‘Whole Family’ approach to meeting their assessed needs.

1.5To identify appropriate intervention / support strategies for children, young people and their families based on their assessed needs including ‘safeguarding’ strategies in partnership with local statutory services.

1.6 To support and to undertake & lead on CAF assessments for children, young people and families referred to the service, and maintain a high standard of case management recording and supervision (in line with Familywise protocols).

PRINCIPAL DUTIES & RESPONSIBILITES

2WORK WITH CHILDREN & YOUNG PEOPLE

2.1Developanddeliver a rangeof innovative1:1 and group work interventionswith children and young people in a variety of settings including detached street work, community bases and schools.

2.2Ensure practice conforms to CANW’s safeguarding policies & procedures.

2.3Encourage andsupport user involvement inTargeted Youth Supportprojects.

2.4Assist in the evaluation and development of the Targeted Youth Supportcontract.

2.5Research and keep up to date with local and national trends affecting children and young people.

2.6To take responsibility for agreed delegated areas of work with the Group Manager.

3LIAISON

3.1Establish & maintain links/relationships with relevant schools, Young People’s Service and partner agencies in the interests of the Targeted Youth Supportproject.

3.2Monitor and record all project statistics and contribute to the preparation & dissemination of information and reports relating to Targeted Youth Support projects.

4STAFF TEAM

4.1Actively participate at working groups, staff meetings and the dissemination of service training as agreed by the Group Manager.

4.2Support the dissemination of information to staff and project users.

4.3Encourage and support the involvement of volunteers.

4.4Ensure the safety of staff, volunteers & project users in line with Health & Safety guidelines.

5ADMINISTRATION, MONITORING & FINANCE

5.1Ensure that reports relating to the Targeted Youth Support project planning and evaluation are undertaken.

5.2Review own performance against agreed annual KPI’s (Key Performance Indicators).

6GENERAL DUTIES

6.1Represent CANW at meetings, conferences and exhibitions as required.

6.2Provide a non-discriminatory service and treat all who access CANW services fairly and equally.

6.3Be aware of the Health and Safety requirements in the workplace, and in particular, as it relates to own service area and/or department.

6.4Work within allocated budgets and agreed expenditure levels.

6.5Keep accurate records and provide written reports as required.

6.6Contribute to CANW’s operational and strategic development, including any organisation objectives identified through designated quality standards, such as PQASSO and Investors in People.

6.7Participate fully in personal supervision meetings and the annual appraisal process and undertake any training deemed necessary for the role.

6.8Review own delivery performance against agreed annual KPIs (key performance indicators), aiming to meet or exceed personal targets/performance levels.

6.9Undertake additional duties, training and/or hours of work as may be reasonably required which are deemed appropriate to the levels of responsibility within the role.

6.10Be aware of the safeguarding policy and procedure for children and vulnerable adults and ensure that the safeguarding of children, young people and adults is given the highest priority in the work undertaken.

6.11Have a personal responsibility to ensure that person identifiable, confidential or sensitive information is processed in line with the Data Protection Act and complies with the law, organisational polices and best practice when processing information.

6.12Work outside of normal office hours (ie evenings and weekends) as and when required to meet the needs of service users as appropriate.

6.13Undertake other appropriate tasks as may be required of the post holder from time to time and provide cover for tasks during periods of sickness or holiday.
