FORM OF CONSENT FOR PONY CLUB MINI CAMP
I ………………………………………….… the parent of ……………………………….……., declare that I have received details of Blackburn and District Branch of the Pony Club Mini Camp 2016. I consent to my son/daughter attending Mini Camp on Tuesday 31 May 2016 to 2nd June 2016 At Somerford Park Farm, Holmes Chapel, Cheshire and taking part in the activities arranged.
I also consent that my son/daughter will follow the rules laid down by the Committee which are made to safeguard the well being of my son/daughter.
I understand that the owners of Somerford Park Farm or Blackburn and District Branch of the Pony Club will accept no liability for any items lost, broken or stolen during my son/daughter’s time at mini camp.
In the event of emergency, I further consent to any medical treatment being administered by qualified practitioners to my son/daughter during his/her time at Mini Camp and also to the administration of any veterinary care to their horse/pony in the case of emergency.

Body Protectors
Pony Club regulations state that a body protector is compulsory for all X-Country riding and is recommended for Show Jumping. At Pony Club Camp we will be wearing Body Protectors for all Jumping activities, unless you wish your child not to wear one for Show Jumping and indicate this by signing the declaration below.
