Biological Science Collegiate Division Fellowship Commitment Form
Initial each of the following to acknowledge that you have read, understand and agree to meet each of the criteria.
1) I am registered for and will attend The University of Chicago campus in Chicago, Illinois in the 2016 Fall Quarter. Yes / No _____
2) I am presently a first year __ second year __ ; Biology Major student Yes/No; Major?______
3) Fellows/Students are not permitted to enroll in any classes at the University of Chicago during the 10-week fellowship. I will not register for any course(s) during the 2018 10-week Fellowship. _____
4) Fellows/Students will conduct 10 weeks of full time research under the supervision of the Principal Investigator from Monday, June 18, 2018and will conclude onFriday, August 24, 2018. _____
5) Fellows/Students will attend an Orientation meeting on Monday June 18, 2018 at noon. _____
6) Fellows/Students will attend and participate in all of the BSCD Principal Investigator Thursday seminar series scheduled during the Fellowship program. _____
7) Fellows/Students attend and present a Research Progress Report session during the fifth (or sixth) week of the Fellowship Program. Fellows will give an oral Power Point presentation of their research progress. These Research Progress Report are 15 minute presentations with 10 minutes for the research presentation and 5 minutes for Q&A.
8) At end of the 10 week program, Friday, August 24, 2018, Fellows must (a) submit a Title and Abstract (via email to Dr. Strieleman, Ms. Kila Roberts and Dr. Zaragoza) for the final presentation at the Biological Sciences Collegiate Division Research Symposium held in the 2018 Fall Quarter. At end of the 10 week program, Friday, August 24, 2018, Fellows must also submit a Summary of their summer research. (via email to Dr. Strieleman, Ms. Kila Roberts and Dr. Zaragoza) _____
9) Attend and present a Power Point presentation of the 10-week summer Fellowship research at the Biological Sciences Collegiate Division Research Fall Symposium to be held on Saturday October 6, 2018, at the end of the first week of the 2018 Fall Quarter. All PIs are invited to attend and are asked to introduce the Fellow and their research project at the 2018 Fall Quarter symposium. A Fellow’s full participation requires that the Fellow present their research and attend the entire/all day symposium, 8:00 am to about 4:30 pm. _____
10) Any publication resulting from the Fellowship research should acknowledge the “The Biological Sciences Collegiate Division Research Endowments at the University of Chicago”. _____
11) Fellows/students that do not FULLY participate as outlined above will be disqualified, dropped from the BSCD Fellowship program and forfeit the entire stipend. _____
I have read, understand and agree to meet all of the above criteria if I am awarded the BSCD Fellowship. I acknowledge that failure to meet any of the above the criteria, at any point during the Fellowship Program, constitutes that I did not fully participate in the program and understand that I will be disqualified and forfeit the entire stipend.
