Consent to Participate Form

I understand that:

I am responsible for my own firearm and ammunition (at least 20-25 boxes of ammo).

We have a mandatory student and parent meeting on Monday, March 6th at 6:00 PM at the Bemidji High School Commons area.

Our shooting nights are at the Bemidji Trap and Skeet Club on Monday’s at 5:00 PM starting April 3rd through Monday, June 12th. As the season progresses, the start time of practice may be adjusted to a later time as daylight allows.

If my child drops out of the club for any reason after the date of March 10, 2017, the trap team fee of $100.00 and the MSHSCTL fee of $35 will not be refunded. The BHS Trap Team Fee includes a new team uniform, a meal each night of shooting, and clay targets throughout the season.

All coaches, scorers, and team managers involved in the 2017 BHS Trap team are volunteers.

We will be asking for parent volunteers to assist with scoring on our shooting nights.

BHS Trap Team should be given priority on our shooting nights. You may not “make up” your rounds. You must shoot on the night that we have designated as our night as per League rules. Please make arrangements with your other commitments.

All participants will be asked to help at various promotional events and be expected to help with fundraising activities.

Shooters will need to provide their own ear and eye protection. High performance shooting glasses with multiple lens combos for different shooting conditions are strongly recommended.

All participants will follow school rules as pertaining to a team or a club. If there are infractions to school rules or BHS Trap Team rules, your child may be asked to leave the team on a temporary or permanent basis.

There is a $35.00 fee due to MN Clay Target League at time of online registration.

Please return this form and your fee of $100.00 to the BHS activities office.

Registration will close on FRIDAY, March 10th. No Exceptions will be made.
