 OPTION FOUR: Completion of Training Sessions Offered at the School or District Level and/or appropriate section of the district proficiency test.

Use the following table to document district or school level training sessions attended where basic competencies were addressed. Teachers may choose to attend training sessions for part of the competencies and then take the test to complete the requirements. See the examples below for clarification.

EXAMPLE ONE: Teacher A attended training sessions at her school on Streamline SC, Integrade, and eChalk. During these training sessions she was able to complete the required tasks listed on the competency checklist, also listed in the Tasks section of this document. She had the training facilitator document that she completed the requirements by signing in the appropriate box in the following table. To complete Phase 1 requirements Teacher A then signed up to take the test in the spring and completed the Functional, Word Processing, ClassXP, Presentation Multimedia, and Internet sections of the test. (She didn’t have to complete the ActivStudio section because she didn’t have a Promethean board.) The test administrator signed the remaining sections and copied the documentation form to turn in to Linda Moya-Mendez at Computer Services. Teacher A kept her copy of the form for documentation.

EXAMPLE TWO: Teacher B attended training sessions at her school on all the competencies listed on the competency checklist and completed the tasks listed in the Tasks section of this document. She had the training facilitator sign off that she had met the requirements. She made a copy of this document for her records and sent a copy to Linda Moya-Mendez at Computer Services.

OPTION FIVE: Completion of Proviso requirements while employed by another school district in South Carolina. Attach a documentation letter from the school district to this form.
