C1 Critical Incident Information ¾ Unique to Massachusetts

Critical Incident

Definition

For an incident to be considered a critical incident, one or more of the following circumstances must apply:

¨  Serious injury or line of duty death

¨  Suicide of a co-worker

¨  Death or serious injury to a child

¨  Prolonged failed rescue

¨  Multi-casualty Incident/ disaster

¨  Victim is know to the responder

¨  Any incident where the personal safety of the responder is jeopardized

¨  Incidents with excessive media interest

¨  Any incident with unusually strong emotional components

Purpose

This coded field will collect data on whether this was an incident with sufficient impact to produce significant emotional reactions in people now or later. This information will be crucial in getting funding and resources for Critical Incident Stress Management Programs as well as identify possible harmful situations to Massachusetts Firefighters.

Team Mobilized

Definition

If a team from the state or regioinal is called to respond to aid the firefighter.

Purpose

This field will collect data on whether this critical incident called for a state or retioinal CISM team to be called to respond.

Circumstances
Definition

This field will collect the reason this incident was classified as a critical incident.

Purpose

To gain a better understanding of the incident, the factors that categorize it as a critical incident are a necessary bit of information.

Codes for Circumstances, list up to three:

1.  Serious injury or line of duty death

2.  Suicide of a co-worker

3.  Death or serious injury to a child

4.  Prolonged failed rescue

5.  Multi-casualty Incident/ disaster

6.  Victim is know to the responder

Example

A fire in a multi-family home kills a family of four; one of the firefighters responding grew up with the father. The Fire Chief calls for a CISM team.

K1 Insurance Information ¾ Unique to Massachusetts

Insurance Company

Definition

The name of the insurance company carrying the owner’s policy.

Purpose

Information required to be collected according to MGL C148, S2.

Entry

This information can be entered as alphanumeric text up to 25 characters in length.

Total Insurance
Definition

Total insurance is the total amount of the insurance policy.

Purpose

This field gives us further information on the property and the owner.

Entry

This is a numeric 10 character field.

Example

The Massachusetts Fair plan insures the owner of the house that burned. The insurance policy is for $100,000.

FIRE MOdule Section H

H Vehicle Stolen ¾ Unique to Massachusetts

Vehicle Stolen

Definition

If the incident involves a motor vehicle fire, this field collects whether the car was stolen when it caught fire.

Purpose

This information is valuable to fire investigators in determining whether the fire is suspicious or incendiary.

The vehicle that was set on fire in a mall parking lot was stolen.

Section H1

H1 HazMat Additions ¾ Unique to Massachusetts

Tier Levels

Definition

This field indicates which level of response from state resources. This field does not apply to Boston, Cambridge, Springfield or other communities that have their own HazMat teams.

Purpose

This field will show the level of the incident.

Entry

The codes for Tier level are:

01. Tier One Hazard & Risk Assessment

02. Tier Two Short Term Operations

03. Tier Three Long Term Operations

04. Tier Four Multiple Team Operations

Number of Entries

Definition

Each 15-minute entry made by each responder should be counted.

Purpose

This field will show the number of entries made by emergency personnel to contain or control a spill.

Entry

This field is a three digit numeric field.

Suit/ PPE Levels

Definition

This field will indicate the level of Personal Protective Equipment used by the responders.

Purpose

This field gives an idea of the severity of the incident.

Entry

The codes for this field are:

01 Level A: fully encapsulating, gas-tight, chemical protective clothing with SCBA or supplied air

02 Level B: fully encapsulating or non-encapsulating chemical protective clothing with SCBA or supplied air

03 Level C: non-encapsulating chemical protective clothing with air purifying respirator

04 Level D: firefighting turnout protective clothing
