Application for (ICATS) Supervisor Module
Instructions and Guidance

1. Contact the Correx Office or visit our Website for available course dates.

2. If the candidate does not currently hold an ICATS card two passport photographs of the candidate must be attached to this application form. The candidate’s name must be printed and signed on the reverse side of the photographs.

3.Upon completion this form, please check that all details are correct, all of the necessary items are provided and signatures are present where requested. Failure to comply with any of the above may result in a postponement or even cancellation of the application. In the event of a false statement being discovered, any certification awarded as a result of the examination will be null and void.

  1. To be considered for the course,

(a)Candidates should haveat least two years’ experience following successfully completing the mandatory ICA ICATS module.

Or

(b)Supervisors and Technical Managers who have more than two years’ experience in the industrial coating field will be considered with supporting evidence from their employer, even if they have not completed the ICA course.

(c)Candidates with at least five years’ verifiable experience in the coating industry will also be eligible.

  1. The fully completed application form and any extra items required in sections 2 and 4 above should be returned to the ICorr Training Centre along with full payment of the course fee.

6.No booking can be considered confirmed until a correctly completed application form has been received, validated and acknowledged by Correx.

  1. The fee for the 2-day course is £675.00 plus VAT,payments by cheque should be made payable to Correx Ltd prior to candidates attendance. Credit/Debit card payments can be made by telephoning the Correx Office on 01604 438222. Candidates who intend to pay their own fees should pay the examination fee in full on receipt of invoice.
