BANDERA 4-H CLOVER BOOSTERS SCHOLARSHIP

2013-14 APPLICATION

I. REQUIREMENTS

  1. An active member of the Bandera County 4-H program for a minimum of 3 years.

B. A high school senior attending Bandera High School, Medina High School, participate in a Bandera County qualified Home School program or Private School program.

C. Must be enrolling in a two-year or four-year curriculum at a college or vocational/technical school. Proof of enrollment is required at time of receipt of award. Recipient will forfeit scholarship and money to be returned to the Bandera 4-H Clover Booster Club’s Scholarship fund if not used within ten (10) months of receipt of award. This allows time for enrollment in either the fall or spring semester of the program you have chosen.

D. Applying Senior must be an active participant in 4-H. (Participation can be verified through Bandera County Extension Office.)

E A minimum 2.5 GPA is required as reflected in official high school transcript.

F.  Two (2) letters of recommendation. Letters may be from teachers, employers, pastors or community leaders who have an insight as to your qualification.

G.  Incomplete applications will be disqualified.

II. APPLICATION PROCESS

A. Application is due at Extension Office by 12:00 noon, April 1, 2014. All applications must be submitted in a sealed envelope, this process is to protect your privacy, and will only be opened by the scholarship committee at the review meeting.

B. Applications will be screened and reviewed by the Scholarship Committee of the Bandera 4-H Clover Booster Club.

C. Applicants will be notified by mail if they will or will not receive an award.

III. SCHOLARSHIP AWARD

A. Acknowledgement of award to be presented at graduation ceremonies or special graduation award night at recipient’s respective high school.

B. Recipient will receive written notification of award amount. Payment of scholarship will be made directly to college/university or vocation school upon receipt of copy of school registration. Recipient is responsible for providing address and paperwork necessary to mail payment.

C. Recognition of Scholarship recipients will be made at the annual 4-H Awards banquet held in August. Recipient and Parents/Guardians are encouraged to attend.

IV. APPLICATION FORM

Please complete and return the scholarship application and required attachments in a sealed envelope to the Bandera County Extension office by 12:00 noon by April 1, 2014.
