If the bird that you found was not clearly photographed, and/or the photographs did not conclusively identify the species, you must fill in this form to give a written description of the bird, and evidence as to why you identified it as this rare species. Please attach to Form A. Also, please copy and attach any notes that you made in the field.
Note that you may not be able to fill in all the sections below – they are just a guide as to what you should try and include. Realistically, in field-conditions, you may not be able to see all these features, and indeed many may be irrelevant to the species involved.

- Plumage :
(please give details of the colouration and patterns on the feathered parts of the bird. It may be easy to split this up into: head, underparts, upperparts, wings and tail.)

(please give details of the colouration of the bill, legs and eye.)

(please describe any calls or songs the bird made.)
- Behaviour :
(please describe how the bird acted during the observation, especially compared to other birds. What did it feed on, where was it feeding, its flight action, where it perched, etc.)
