Annual Report

This annual report describes the goals that were worked on and what was completed in 2015 by the membership of the AZ Health Care Coalition South Region.

AzHCC South Region

Executive Policy Board

  • Develop annual report format and date for submission to ADHS, Membership
  • Maintain by laws
  • Determine AZCHER South HCC membership leadership requirements
  • Evaluate legal and financial requirements for AZCHER South HCC
  • Evaluate effectiveness of training and exercise program
  • Promote, evaluate and maintain strategic planning for AZCHER South HCC
  • Approve AZCHER South HCC leadership elections and Strategic plan
  • Board started but is still working on:
  • evaluating regional documents to be sure all sector, rural and reservation agencies are incorporated into plans and exercises

AzHCC South Region

  • Ensure partner roster is up to date
  • Expand membership to capture existing potential partner agencies not currently participating
  • Develop and provide year-long education plan that will be provided in 2015 and then will be available online as an introduction to HCC members
  • 100% participation (from eligible parties) in the MOU
  • Complete regulatory and accreditation requirements crosswalk
  • Complete Regional HVA
  • Establish and maintain Strategic planning date annually to ensure effective planning strategies
  • AZ HCC South Region membership started and is still working on:
  • Develop healthcare resource coordination center process
  • Develop healthcare resource 96 hour sustainability inventory
  • Develop regional healthcare reception site / reunification plan
  • Complete regional plans – EVD, infectious disease, Mass fatality

AzHCC South Region

SECTORS

Ambulatory Care – Tracy Kruze

  • Assign sector member to be part of training and exercise committee
  • Incorporate ambulatory care agency involvement in exercises
  • Review regulatory requirements and determine methods to meet requirements that can be adopted by all agencies
  • AZ HCC South Region membership started and is still working on:
  • Complete process to elect co chair

Behavioral Health

  • Examine Joint Commission and other accreditation standards for 2016 related to outpatient requirements for behavioral health providers to participate in Emergency Preparedness activities.
  • Ongoing training in the area of Integrated Behavioral Health. Stressed importance of communication and coordination of care.
  • Evaluate the training needs within the behavioral health system in regards to emergency preparedness.
  • Amend Memorandum of Understanding (MOU) to be inclusive of integrated health care (medical and behavioral health).
  • AZ HCC South Region membership started and is still working on:
  • Enhance provider awareness and involvement in Emergency Preparedness table top exercise.
  • Develop cover letter for survey and distribute the survey community wide (include SNF’s, etc.) to gather information as to available resources in the event of a disaster – Asset Inventory.
  • FEMA compliance - update CPG 101 (Community Preparedness Guide) and SLG 101 (State & Local Guidelines for 1st Responders) to include behavioral health input. Document incorporates both federal and state/local victim services.

Community Resources – Steve and Cheryl

  • Identify community resources
  • Determine method for bringing all community resources to table in event
  • Build community call center operation
  • AZ HCC South Region membership started and is still working on:
  • Build pre credentialed volunteers
  • Develop volunteer coordinator center process and staffing

Correctional Health

  • Coordinate opportunities for correctional health partners to participate in exercises and drills.
  • AZ HCC South Region membership started and is still working on:
  • Solicit HVAs from partners for incorporation into the regional HVA.
  • Assemble an Asset Inventory for correctional health partners.
  • Rebuild correctional health involvement in the coalition.

County Public Health – Louie

  • Define Sector role
  • Develop All County Public Health Department communications strategy for disaster
  • Develop Strategic plan with AzCHER South HCC Chair and Co-Chair and define goals
  • Work with counties to build border response and intelligence network
  • AZ HCC South Region membership started and is still working on:
  • Complete regional plans

Hospice/Home Care

  • AZ HCC South Region membership started and is still working on:
  • Assemble list of potential partners from State licensing body
  • Identify Sector Chair
  • Support regular meetings
  • Draft goals for sector

Hospitals

  • AZ HCC South Region membership started and is still working on:
  • Identify Sector Chair and co chair
  • Support regular meetings
  • Develop healthcare resource coordination center process
  • Develop healthcare resource 96 hour sustainability inventory
  • Develop and test hospital reception site/ reunification plan

Long Term Care – Richard and Paula

  • Attend exercise and training committee meetings to incorporate Long term care and skilled nursing agencies in all exercises
  • Draft goals for Sector
  • AZ HCC South Region membership started and is still working on:
  • Develop and expand membership in sector
  • Maintain education on HICS for long term care
  • Review and determine methods for long term care and skilled nursing to meet new accreditation requirements

AzHCC South Region

COMMITTEES

Communications

  • Use AZHAN as the primary emergency notification system for the Az HCC South Region
  • To that end, continued emphasis on getting members to sign up
  • Complete AZHAN communication drill annually
  • Develop regional communications plan for Az HCC South

Website/Logo

  • Complete publish-ready logo
  • Set additional website pages
  • Determine locked page strategy

Credentialing

  • AZ HCC South Region membership started and is still working on:
  • Complete Physician credentialing plan
  • Communicate Physician credentialing plan to statewide association for adoption
  • Develop human resources credentialing plan

Strategic Planning

  • AZ HCC South Region membership started and is still working on:
  • Set strategic plan for the next 10 years
  • Review nonprofit status, consider other options

Policies and Procedures

  • Assist Communications committee with a regional communications plan
  • AZ HCC South Region membership started and is still working on:
  • Review Mass Fatalities Plan for regional plan
  • Build regional EVD/infectious disease plan

Regional HVA/Asset Inventory

  • Build Regional HVA
  • AZ HCC South Region membership started and is still working on:
  • Build regional 96 hour sustainability chart with an extensive Asset Inventory Database
  • Review database for items that do not rank high enough on 96 Hour chart

Training and Exercise

  • To train membership on planning, preparing for, and participating in a regional full scale exercise by executing a progressive series of educational opportunities and coalition-wide drills and exercises using regional HVA
  • To hold a TEPW and from the information gathered, develop a 3 year MYTEP to be published and shared with the HCC
  • To continue to make relevant trainings available through the website
