NARRATIVE(Limit to two pages)

HM3 Sailor has clearly distinguished himself through his exceptional performance as the senior Radiation Health Technician aboard IKE, who is directly responsible for administrative oversight of a nearly 700-person Radiation Health Program. As Work Center Supervisor, he directed the audits and screenings of all radiation health medical records to ensure efficient monitoring of the health and safety of all watch standers, fire party, and damage control personnel in and around reactor spaces.

Petty Officer Sailor continually devotes countless off-duty hours meticulously reviewing, updating, and maintaining nearly 700 medical records for IKE’s Radiation Health Program. When an unforeseen manpower shortage left him as our sole Radiation Health Technician for six months, he maintained this extremely demanding program with absolutely no degradation in quality or performance. Through innovative forward thinking, he developed a database to track recurring medical requirements for 450 radiation workers. His utilization of this database, along with his close attention to detail in all aspects of the program, resulted in a Reactor Safeguards Examination grade “well-above” the COMNAVAIRLANT Fleet average and cited as “Best in the fleet.”

HM3 Sailor volunteered to go TAD to support the USS ENTERPRISE (CVN 65) during a two-month underway period. During this time, he completed a 100% medical record audit of the Radiation Health Program, effectively applying his professional knowledge and expertise by identifying and correcting 150 medical record discrepancies. This audit was instrumental to their being graded as “Above Average” during their Operational Reactor Safeguard Examination. HM3 Sailor was awarded a NAM for his contribution.

HM3 Sailor diligently worked to complete the final installation of the digital radiography network system in support of IKE’s RadiologyWorkCenter. This tasking required expert coordination among Medical, Combat Systems, National Naval Medical Center (NNMC), and a civilian contractor. The system enables IKE to digitally transfer electronic radiographs to a certified radiologist at NNMC for diagnosis. This rapid transmission significantly decreases the wait time for a definitive diagnosis of serious fractures and/or internal injuries, and expedites implementation of appropriate medical treatment. He was awarded IKE’s Hospital Corpsman of the Month for October 2004.

HM3 Sailor installed the Snap Automated Medical Systems and Composite Health Care Systems databases on all computers within the medical department; his initiative and efforts directly improved efficiency by providing 100% access for all medical personnel. He designed and implemented a pharmacy drug locator database, reducing patient wait times for medications and refills. As the Department ISSO (Information Systems Security Officer) he provides invaluable one-on-one training and assistance in office computer applications for medical department personnel. As IKE’s Medical Representative for the Command Security Clearance Program, HM3 Sailor screens 100% of all IKE medical records for personnel requesting or requiring a Security Clearance at his command.

HM3 Sailor’s pride, professionalism and dedication in his primary and collateral duties place him well above his peers. His enthusiasm and attention to detail in the execution of every assignment inspires confidence in his subordinates, peers, and superiors alike. He also successfully maintained his command personal qualification objectives and is currently striving toward his personal goals by completing Introduction to Literary English; Introduction to Computer Information Systems; and Concepts in Information Technology. These courses were completed in pursuit of a Bachelor of Science in Computer Information Systems.

HM3 Sailor is the walking embodiment of Honor, Courage, and Commitment. This exemplary Sailor consistently maintains an impeccable uniform appearance, imbues exceptional medical and technical experience, professional confidence, and the maturity required in more senior Petty Officers. He is without question the hardest working Radiation Health Technician in the Navy.

BIOGRAPHY(Limit to one page)

HM3 Sailor was born in Colorado Springs, Colorado. As a child his family moved to Texas where he spent most of his life up until completion of high school. After high school he moved back to Colorado with his family where he entered the medical field as a Certified Nursing Assistant working primarily with the elderly and disabled.

In 1997 HM3 Sailor enlisted into the delayed entry program and left for boot camp in 1998. Upon completion of boot camp he attended Hospital Corpsman A-School from April 1998 until September 1998. In September 1998 he went on to RadiationHealthTechnicianSchool where he scored the highest overall average in his class upon graduation with a 97.1% average.

After graduating C-School in November of 1998 he went on to Naval Hospital Roosevelt Roads and tenured an independent billet until 2002. In September of 2002 he transferred to USS Dwight D. Eisenhower where he remains currently.

HM3 Sailor is married to Mrs. Sailor and has three children, Alpha, Beta, and Gamma. He has been awarded two Navy-Marine Corps Achievement Medals, one Meritorious Unit Commendation, one Navy-Marine Corps Good Conduct Medal, the National Defense Service Medal, three Navy-Marine Corps Overseas Service ribbons, and the Navy Expert Pistol medal.
