Selected flight schools mustuse this form as part of their student application and selection process, and securely maintain it during the lifetime of the contract, to ensure students meet the minimum requirements to satisfy the government’s objectives of the Forces to Flyer initiative. All students will be required to complete this form as part of their application process to selected flight school.

- I understand that the Forces to Flyers initiative will not cover approximately $13,526 in training costs that will be incurred within my first 12 months of training and that I, with or without assistance from my training school, must provide these funds. I understand that $13,526 is the equivalent of the 100% benefit levelof Post-9/11 GI Bill funding for one (1) academic year of vocational flight training. I may pursue this option to cover the $13,526 in costs or I may pursue other options including using my own funds.The flight school has alsoinformed meof anyadditional costs associated with flight training for which I will be responsible.
