Reports to: Supervisor – Swissvale In Home Office
Hours: Full-time. Work evenings and some weekends. Must show flexibility in scheduling to satisfy the needs of the clients.
General Statement of Duties: The Assessment Specialist serves as the initial point of contact and completes a comprehensive assessment with families referred to the In-Home program. The Assessment Specialist works collaboratively with counselors assigned to families and with the referral agent, Allegheny County Children Youth and Families. The Assessment Specialist Services are provided in the family’s home.
Essential Functions:
1. Promotes the Mission and Philosophy of Holy Family Institute.
2. Provides formal assessment of family strengths, problem areas, levels of risk to the children, and community resources.
3. Must be able to recognize signs and symptoms of addiction, mental health, and developmental concerns
4. Develops and implements goal-based services to families.
5. Provides counseling, family education, and case management services to families.
6. Documents all progress of services provided.
7. Work cooperatively with CYF and staff members to insure quality service to each family.
8. Maintains a positive working relationship with families and fellow staff.
9. Must meet CYF service delivery time frames for proper intake and client discharge.
10. Be available to families in the district for crisis situation.
11. Develops skills training in working with families and children.
12. Demonstrates computer skills utilizing Microsoft office and office 365.
Minimum Qualifications:
Candidate must have a Master’s Degree in Social Work or related field with one to three years’ experience. All services are provided to clients in-home during working hours with occasional availability for evening and weekends. Must have reliable transportation and use of a car during working hours. Valid PA driver’s license and Act 33/34 and FBI clearances are required. EOE
