Assessment Project Checklist

This checklist is designed to help CSU interns, mentor teachers and supervisors ensure that interns completed all of the steps and products required in the assessment project.

1. ___ Completed the Class Profile form.

2. ___ Chose a focus for the curriculum project or unit.

3. ___ Wrote a well thought out written rationale for choosing this project or unit.

4. ___ Got your mentor teacher’s written approval for the project or unit.

5. ___ Taking the students and class context into account, wrote four outcomes that students were expected to achieve through this curriculum project or unit:

a. ___ One knowledge outcome,

b. ___One skill or process outcome,

c. ___ One disposition outcome, and

d. ___ One emotion outcome.

Regarding 5a-5d above, ensured that the following expectations are met.

e. ___ All four outcomes were appropriate for these students.

f. ___ All four outcomes were truly important.

g. ___ Students and/or students’ families chose at least one of the four outcomes.

h. ___ At least two of the outcomes relate to under-emphasized subjects or developmental

domains (motivation, social development , moral, physical education, the arts).

6. ___ Wrote a separate rationale for each outcome, explaining why it was important to achieve.

7. ___ Listed the benchmarks and/or grade level indicators that each outcome relates to (use Ohio’s benchmarks where they exist; use national standards for other areas).

8. ___ Got written approval of these outcomes from your mentor teacher and university supervisor.

9. ___ Wrote a separate assessment plan for each of the four outcomes.

10. ___ Reviewed these assessment plans with your mentor teacher.

11. ___ Revised the assessment plans based on feedback from your mentor teacher.

12. ___ Wrote a detailed plan for the teaching methods to be used during the project or unit.

13. ___ Submitted your teaching plans to your mentor teacher and university supervisor for written approval, at least one week prior to the beginning of the project/unit.

14. ___ Revised your teaching plans based on feedback received.

15. ___ Conducted the pre-assessments for each outcome for which they were needed.

16. ___ Implemented the project or unit, carefully considering the students and the context.

17. ___ Conducted the post-assessments for each of the four outcomes.

18. ___ Completed a written report on and analysis of anticipated and unanticipated student

outcomes.

19. ___ Completed written reflections on other lessons learned from carrying out this curriculum project or unit.

20. ___ Completed this assessment project checklist, to ensure that you completed everything!
