Assessment Coordinator:
Thinking Ahead
Responsible to / Service ManagerSalary / £24,547 per annum (Agenda for Change Band 5 point 19)
Hours per week / Full time
Annual Leave per annum / 25 days pro rata (rising to 30 days after 5 years)

Main aims of the post
The post-holder will work within the Thinking Ahead Wellbeing Service. The post-holder will work alongside the management team to provide coordination of the triage and assessment process within the service. The post-holder will focus on assessing individuals accessing the service and providing a thorough and holistic assessment of the individual’s needs, assigning them to the most appropriate service. This work may be face to face, telephone, group, or via other media. The post-holder will be responsible for managing their caseload and providing key performance information as required.
You will need to hold a counselling or primary mental health care qualification and be in possession of excellent assessment and communication skills. You will demonstrate a minimum of 2 years post graduate experience of working within a Mental Health Service Provider.
Main duties of the post
- To use a range of evidence based tools and techniques for assessments, goals and motivating clients
- To manage a varied caseload of clients
- To work with groups of clients to help them achieve their goals
- To collate and analyse performance data as necessary
- To develop effective relationships with a range of external organisations and agencies
- To promote the service to communities and agencies as appropriate
- To keep accurate and up to date records of clients records
- To ensure quality standards are adhered to and met and that audits are passed
- To ensure all statutory responsibilities are followed and reported as required
10.To represent the team and organisation at external events
11.Toprovide comprehensive, time limited assessments to clients. This work may be face to face, telephone, group, or via other media.
13. To triage referrals for Thinking Ahead, screening for risk/safeguarding or complexity issues and to manage them appropriately by adhering to organisational policies.
