References

Taking references helps us ensure that you are suited to the volunteering role you are interested in. Please provide references for two people over 18 that will be able to tell us about you. These can be neighbours, colleagues or close friends, however if possible one should be someone you know in a professional capacity.

Under the rehabilitation of Offenders Act 1974, do you have any unspent criminal convictions? YES/NO

(If yes, please summarise on a separate sheet. Having a conviction will not necessarily stop you from volunteering, but will need to be taken into consideration when assessing your suitability.)

DBS check (Previously CRB)

Depending on your voluntary role within the organisation, it may be that you have to undergo an enhanced disclosure through the Disclosure and Barring Service (DBS, previously CRB). This is because you may have contact with children and vulnerable members of the community.

SIGNATURE DATE

This registration form reflects the hopes and intentions of the volunteer and the Tullie House Museum and Art Gallery Trust and is not contractually binding in any way, nor does it constitute a contract of employment, now or in the future. The volunteering may be cancelled by either party at any time.

The information you supply will only be used to assess your suitability for the role applied for. Where a speculative application is made we will add your details to a computer database and use your contact details to inform you of upcoming opportunities. We will keep your information and forms for nine months. Any details supplied will only be disclosed to the Volunteer Co-ordinator, the role supervisor or administrator. Should you take on a volunteer role references will be checked and the form will be retained until after you cancel your volunteering role or for as long as legislation dictates.
