I, the undersigned, declare that I:
  • apply for the exemption of the horse(s) listed in the Schedule from the veterinary examination required by Section 40 of the Animal Health Act 1981;
  • understand that if exemption from veterinary examination is granted, this application will also be treated as an application for the veterinary inspection required by Article 7 of the Export of Horses (Protection) Order 1969 or for the exemption from that inspection;
  • understand that the horse(s) will require to be rested before shipment at approved premises* in accordance with the Export of Horses (Protection) Order 1969 unless the licence issued in response to this application grants exemption from that requirement. I acknowledge that if such an exemption is not granted I am responsible for making arrangements at my own expense for the detention of the horse(s) at approved premises;
  • declare that to the best of my knowledge and belief the particulars given above and in the Schedule are correct;
  • understand that if a licence is issued this does not confer any consent or approval that maybe required under any other regulations in connection with this export.
