Note:

  1. Franking of the Member sub-broker agreement will be done by HO.
  2. Three Signatures of Applicant are required on 1st Page& One Signature is required on all remaining pages of Agreement by all the partners along with stamp
  3. All Partners Signature (with Stamp) required on Recognition Form, Form B, Undertakings, Annexure L and Agreement
  4. All proof submitted should be duly attested by a C.A or a Notary.
  5. All certification by a C.A should consist of his signature, stamp & membership no
  6. Experience certificate in Capital Market as an employee/dealer for more than 2 years.
  7. Don’t submit for joint a/c’s & Without mentioning S/o, D/o, W/o, C/o., ICICI, HDFC & Axis Banks allowed
  8. In case of any Address Proofsubmitted – Pin Code is mandatory.
  9. In case of partnership applicant, any partner is a Married female and there is change of name, then, copy of marriage certificate / affidavit duly attested by notary supporting change of name should be submitted.(refer Formats)
