Application for General Health Care Professional Scholarship

The All Saints Foundation Health Care Professional Scholarship is offered to current associates seeking a career in health care or advancement in their health care profession. Through generous donations to All Saints Foundation, we are pleased to afford our associates this opportunity for financial support in accordance with our value of Development.

Scholarship Criteria*:

  1. Applicant must be a current All Saints employee in good standing and enrolled in a health care professional program. *Not applicable for nursing degree
  2. Financial circumstances indicating need will be considered.
  3. The scholarship recipient must provide proof of enrollment in an accredited health care professional program
  4. Longevity with All Saints; must be employed and have active status for at least one year
  5. Must be in good standing, free of written warnings for at least 1 year
  6. Financial circumstances indicating possible need
  7. Scholastic Performance
  8. Community and organizational volunteerism
  9. The recipient is expected to remain employee at All Saints at least during the timeframe of the award, longer is preferred.
  10. In no event shall there be any restrictions based on race, creed, color, or sex of candidates.
  11. Applicant must submit six copies of entire application
  12. All applications must be in typed format, hand-written applications will not be considered.

*Any exception to these criteria may be reviewed and considered by the Scholarship Committee.

Scholarship Process:

  1. Scholarship applications available via TauNet, or call All Saints Foundation at 262.687.8654.
  2. The scholarship recipient may be asked to meet with the respective scholarship donor and will be asked to attend a scholarship awards reception, tentatively scheduled for March 2019.

Required Materials for Submission:

Two letters of recommendation

  • One from supervisor, manager or director within All Saints
  • General character reference (not a relative)

College transcripts showing academic performance in recent college coursework

Typed application

Typed essay not exceeding one page

Six copies of entire application

Signature of applicant
