Instructions for applicants

Applicants should prepare documents using formats downloaded from our website.

1. Curriculum vitae(Form 1)

(1) Write down your educational background after undergraduate with names of departments and universities in the “Higher Education” column.

(2)Write down your academic careers with names of departments, institutes and positionsin the “Work Experience” column.

2. List of achievements (Form 2-1, 2-2, 2-3, 2-4, 2-5)

(1) List original papers, review papers, bookswritten in English and proceedings ofinternational conferences in Form 2-1.Articles written in other languages and presentations at local conferences should be listed in Form 2-2.Number them in chronological order.

A. Original papers

1)Double underline the title of your academic dissertation.Double overline title of your SCI / SSCI / A&HCI dissertation.Should be classified as peer review ornot.

2) Papers under submission should not be included. Papers in press can be listed with

verifications.

3) Abstracts of conferences should not be filled in.

4) Fill in Impact Factors indexed in the Journal Citation Reports in 2015 by Thomson

Reuters for each original paper.

B.Review papers

1) Papers under submission should not be included. Papers in press can be listed with

verifications.

2) Fill in Impact Factors referring the Journal Citation Reports in 2015 by Thomson Reuters

for each paper.

C.Books

List all authors’ names for co-authored books. In the case of the sharing writing, specifywhich section you contributed. In a case of the written translation, both of original and translatedtitles should be described.

D.Proceedings of international conferences, etc.

List keynote lectures, invited lectures, educational lectures orsymposiums, etc. besides experiencesas chairperson.

(2) Summary of major articles (Form 2-3)

Pick up 10 papers representing your achievements in Form 2-1 and 2-2, and underline the listed numbers. Write each summary in 100-200 words and number of citations with the referenced database and the search date.

(3) Achievements of clinical trials (Form 2-4)

(4) Award, registered society (name and title) and social contributions (Form 2-5)

Write in the following sections and orders.

B. Registered society and your title

C. Social contributions

3. Overview and aspirations for research (Form 3)

Write past achievements and future perspectives of your researchin approximately 600 and 400 words, respectively.

4. Overview and aspirations for education (Form 4)

Write past achievements and future perspectives of your educational activitiesin approximately 600 and 400 words, respectively.

5. Overview and aspirations for clinical practice (Form 5)

Write past achievements and future perspectives of your clinical achievements in approximately 600 and 400 words, respectively.

6.Acquisition of external funds (Form 6)

Fill in the name, period, research theme, distinction of roll (representative or collaborator), and allotted amount of money of the external funds in chronological order.

7.Additional Remarks

Submit these forms and verificationsso that we can make copies directly. When you use booklets as verifications, submit copies of them. You can copy forms if you need more space.
