APEX WINTER FIELD LACROSSE CAMP
Apex Lacrosse Club is pleased to announce the return of our popular winter field lacrosse camp at beautiful Commonwealth Field House. Receive coaching from top rated coaches and NCAA players in the fastest growing game in North America. Come join us in an affordable, fun winter camp environment. The camp is intended for advanced lacrosse players only. Registration will be capped to ensure quality. The camp will sell out so register early.
This year we will hold five 2 hour sessions held on:

Waiver and Release
I/We the Parent/Guardian of the above named registrant are fully aware of and appreciate the risks, including the risks of catastrophic injury, paralysis and even death, as well as other damages and losses, associated with my child’s participation in lacrosse and related activities. I agree on behalf of myself, my heirs and personal representatives that Apex Lacrosse Club Inc.(“Apex”) Greater Edmonton Lacrosse Council (“GELC”), the City of Edmonton, the Alberta Lacrosse Association (“ALA”) together with the directors, officers, coaches, officials, volunteers, employees, agents of same and any sponsors shall not be held liable for any injury, loss of life or other loss or damage as a result of my child’s participation with Apex.
I / We hereby give my/our approval of the above named registrant’s participation in all Apex activities. I / We assume all risks and hazards to his participation in these activities as well as all risks and hazards incidental to the activities and transportation to and from the activities. I / We hereby release, absolve, indemnify and hold harmless all directors, officers, organizers, coaches, managers, agents, partners and officials dealing with or appointed by Apex from any cause of action. I / We likewise release from responsibility any person transporting our child to and from the activities to the extent not covered by liability insurance. I / We will furnish proof of birth date of the above registrant upon request of Apex. I / We understand all of the above registration statements and agree to abide by all the rules and regulations of Apex. Participants in Apex may have their image, likeness, name, province, city , competitive event or coaching history used in publications and on the website or social media by Apex as well as its agents and sponsors from time to time. When signing this form, players, volunteers, coaches, officials and their parent(s)/guardian(s), agree that they have the authority to provide this authorization/approval to Apex and its agents. By completing this registration form I/we acknowledge and agree to the above Apex Waiver and Release.
