ANR New Hire Checklist

____Reminder: You must contact Emily Schutzman in the ANR Business Operations Center at

530-750-1297 or at when it becomes time to plan the move in order to ensure that expenses are allowed by UC Policy and/or established practice(s) in advance of making your relocation plans.

____New Employee Personal Information

____State Oath Allegiance, Patent Policy and Patent Acknowledgment

(Supervisor or Office Manager needs to witness the employee’s signature and sign form)

Oath:

  • To be signed by US citizens only
  • Employee must sign on or before the first working day

Patent:

  • All employees must sign (citizens and non-citizens)

____Employment Eligibility Verification (Form I-9)

(Supervisor or Office Manager needs to review and verify documents and sign form. Please DO NOT photocopy

or retain copies of documents presented by employee.)

  • Employee must sign within 3 business days of first working day

____UC Employee’s Federal-State Withholding Allowance Certificate (W-4)

____UC Nonresident Alien Employee’s Federal-State Withholding Allowance Certificate (W-4)

Directions for Paperwork:

  1. Please fill out the Academic Personnel Action Form (APA Form), Section V. Citizenship/VISA, and sign and date the form.
  1. Please scan and password protect the employment paperwork and APA Form as one file in PDF format. Below are instructions on how to set up password protection in PDF.

  1. Email password protected file (this includes the Academic Personnel Action Form the supervisor signed and completed employment forms) within 3 business days tothe appropriate contact person at the Business Operations Center below. Immediately after sending the file, send a separate email with the password.

  1. Once you forward the forms and the BOC enters the employment information into the payroll/personnel system, they will send the academic’s UC employeeID number to the supervisor via e-mail. The academic can then, if eligible, enroll in benefits and obtain email accounts.

Enrolling in Benefits:

  1. If eligible, your new academic can learn about and sign up for benefits at:
  1. The period of initial eligibility (PIE) is 31 days from date of employment to enroll in benefits.

Setting Up Email Accounts:

  1. The academicshould sign up for an e-mail account (if the headquarter is located at Davis, County or a REC)at by clicking on “Get your UC Davis Computing Account.” If the academic isheadquartered at UCOP, s/he will get a UCOP email address instead.
  1. Once a UC Davis email account is created, s/hewill need to obtain a ucanr.edu email address. Unfortunately, this is a “manual” process. To get a ucanr.edu email address, please have the academic contact Damon DiPietro @ and he will initiate the new email process.
  1. Your new academic has been added to the UC ANR Directory. Once s/he gains access to the

ANR Portal, please ask your new hire to search for his/her name and edit their information at
