As a full member of AMED, you are invited to our Annual General Meeting on Wednesday 14th May 2014 at 1.00pm.

If you are unable to attend, we would be very grateful if you would complete and return by 1st May this Proxy Form to ensure a quorum for the meeting. It’s also an opportunity for you to influence the direction that AMED is about to take.
