Annual Department Action Plan (DAP)
This form provides a documentation template for annual Department Action Plans (DAPs) designed to assess unit outcomes that are designed to impact the learning environment and improve the educational programs and processes of VCC. Initial information and projections describe the PLANNING PHASE of the process. At the conclusion of the project/activity time frame initial information and projections will be updated by completing an EVALUATION PHASE including modifications, outputs and outcomes. Both planning and evaluation information will be organized according to parallel criteria including: (1) a formal goal statement, (2) outcome measures, (3) collaboration with stakeholders, (4) evaluation methods and (5) use of results for improvement of unit performance in meeting established outcomes. The overall process will effectively review and document specific measures of educational effectiveness.

1. Goal-principal purpose and objectives of plan / OSD will collaborate with Standardized Testing regarding the collection of contact information for students who self-disclose their disability on the CPT pre-screening questions. / Directors of Standardized Testing and the Office of Students with Disabilities coordinated the transfer of CPT students’ disability disclosure.
2. Outcome Measures-how plan will be reviewed and measured outcomes / 1. Contact made with incoming students to provide information about OSD’s existence, services, and registration procedures.
2. Determine if information disclosed on CPT is regarding appropriately documented disability.
3. Revisit questions to determine if they ask appropriate questions. /
- When available, list of students indicating disability sent to OSD for follow up contact.
- Contact made with students who had valid information.
- OSD did not determine if students disclosing a disability on the CPT did or did not have a documented disability that required OSD services

OSD will determine if questions are appropriate and useful. / Letters were mailed to students letting them know that the letter was sent because they self-disclosed disability information when they took the CPT. The letter provided general information about the OSD including location and contact information.
No response from students was accurately tracked. We will devise a better way to follow up.
5. Use of Results-how plan will/has
impacted learning environment of VCC and improvement of the educational programs and/or processes / Provides personal communication to students who have disclosed a disability. / Student outreach - students who may not have known about our services were provided information so they can make informed decisions about seeking services.
We plan to tweak the process (tracking and follow up) and continue for the next year.
