NOTE: Recognition by the Rho Chi Society is not relevant to this leadership recognition. Include Rho Chi membership in the Prescott Scholarship Report, not in the Leadership Report. Note: Rho Chi Leadership positions (e.g., president) may be included in this report. Professional and service projects should be excluded from this report and described in other relevant reports.

DESCRIPTION:
The Leadership report was established in 1995 at the 60th Grand Council in Memphis, TN. This is a report of the individual and collective Leader-Development and innovative activities by Phi Delta Chi Chapters to promote leadership and improve leadership skills amongst the Brothers. This is to augment the programs Phi Delta Chi has developed in Leader-Development and to show why we are known as “Leaders in Pharmacy.” The award was renamed at the 65th Grand Council in San Antonio after one of the most remarkable Brothers the Fraternity has ever seen. John D. Grabenstein has selflessly offered his leadership to the Fraternity for over three decades. His dedication and commitment are a shining example to the Fraternity and the world of pharmacy (as the “father” of vaccinating pharmacists). There is no more deserving Brother and his example is something that we should all aspire to live up to.
WHAT TO SUBMIT:
Your report should discuss events that have occurred between December 2016 - November 2017. The judges suggest that you include the following according to the stated criteria:
Programs promoting Leader-Development and People Skills
Describe Leader-Development programs, which your Chapter implemented to improve the leadership capabilities of your prospective members, Brothers and other students at your campus. Include training programs, activities and any innovation your Chapter has developed. Indicate how many or what fraction of Brother participated in each program. State the number of people served by these programs. Remember that leadership is the ability to influence others to accomplish a task. Only describe programs that increased the ability of people to influence others; do not include programs where leaders simply exerted leadership (all programs have that, so you wouldn’t be describing anything remarkable). If it’s a scholastic project, describe it elsewhere, not in this report. If it’s a professional project, describe it elsewhere, not in this report.
Chapter recognition of Leader-Development and People Skills
Describe how your Chapter recognizes the leadership achievement of prospective members, Brothers (Collegiate and Alumni) and other pharmacy students through awards, incentives, or other forms of recognition.
Outcomes of Leader-Development by the Chapter
Quantitative measure of Chapter’s leadership achievement. Include number of Brothers on the roster of Phi Lambda Sigma or Omicron Delta Kappa, officers in other pharmacy organizations (do not list Phi Delta Chi chapter offices), pharmacy student government, University student government, receiving honors or awards based on leadership traits, in comparison to total Chapter membership. Do not include scholarships based on GPA or need, only on leadership. Scholarships listed in the Prescott Scholarship Report should not be listed. Furthermore, only list leadership roles in pharmacy or university organizations, or awards received for Chapter projects and endeavors. Do not include Rho Chi membership unless it relates to a specific office held, lest the judges conclude you haven’t read the instructions.
Written Presentation
Brevity is the hallmark of a well-written report. Do not let the content of the report exceed 6 pages in length, this limit does not include cover pages, table of contents, pictures, etc. The entire report should be no more than 10 pages. The judges are looking for a professional and concisely written report describing your activities. Creative programs and leadership success are more important than creative writing. Note: If including an image of the crest aka coat of arms in your report, please use the official version found under Collegiate Resources at

Item #17: Founders' Day Activity with Photograph (2 points total)
Phi Delta Chi Bylaws, Chapter III, Number 8: "November 2nd shall be known as Founders’ Day and shall be observed with a fitting ceremony by every Chapter, both active and alumni." Suggestions for the event include a Founders' Day Banquet or Luncheon with a guest speaker, a seminar with a pharmaceutical topic, planting a tree, or a memorial ceremony honoring the 11 Founding Fathers.
“Regional” Founder’s Day celebrations have become popular in the last few years. Unless your chapter is hosting this type of event or a large majority of your Chapter’s Collegiate Brothers are attending this type of an event, the Chapter is still encouraged to plan a local Founder’s Day celebration so that Brothers who are unable to attend the “Regional” celebration can still have a chance to observe the day.
If possible, attach a high resolution digital photograph(s) of your Founders' Day Activity suitable for printing in The Communicator. Captions should be listed below with the names of the Brothers pictured. Please send the highest resolution photo possible. For print publication, it is best to send the new, unedited file directly off the camera. Try not to save the photo using an image handling program such as iPhoto, Facebook or others as they reduce the file size and quality. Any photo file format will be accepted. Do not include photos that show alcohol or unprofessional behavior. Treat the memorial with respect.

ITEM #18: Formal Meeting with Regalia (1 point)
According to Chapter IV, Number 12 of the Constitution and Bylaws of Phi Delta Chi Fraternity, "Each Chapter shall have at least one formal meeting each month." Formal meetings require use of the complete Ritual opening and closing, as well as Officer's Robes, Regalia, and devices as outlined in the Ritual Book. ITEM #19: Outstanding Chapter Advisor (1 Bonus Point)
Recognizing the significant role performed by Chapter Advisors and the importance of the continuity which they provide our Chapters, the Executive Council has established an annual recognition to be known as the Outstanding Chapter Advisor Award.
If your Chapter wishes to nominate your Advisor for this award, complete the bottom portion of this page and upload a one-page maximum, typewritten nomination document as instructed below for full credit. Remember to save a copy for your Chapter’s records. If your Chapter has more than one Advisor, please nominate only one of the Advisors. Multiple advisor nominations will not be considered for judging.
The 5 most recent recipients will be considered ineligible for the award. Chapters whose advisor has been honored with this award within the last five years should not re-nominate the same advisor or submit another advisors name, and will receive the full credit for this item, so that other Chapters advisors can be recognized for their service.
If your Chapter chooses NOT to nominate your Advisor for this award, no Bonus points will be awarded.
This award will be presented during the Fraternity's annual awards ceremony. The judging panel will consist of the 5 most recent recipients of the award under the review of the executive council.
Chapters who wish to nominate a Chapter Advisor should submit a one-pagemaximum, (double-spaced) typewritten statement indicating why you believe your Chapter Advisor is worthy of this award. The Chapter should consider both ways in which the Advisor has gone above and beyond for the Chapter as well as exemplary professional, leadership, and scholastic attributes of the Advisor. Below you will find the Job Description of the role of Chapter Advisor. A nomination for Outstanding Chapter Advisor should describe how the Advisor goes beyond the duties outlined below.
JOB DESCRIPTION:
The very term "advisor" implies the basic function of the position and its relationship with the Chapter. It is not the Advisor's role to run the Chapter. Rather, the Advisor's role is more of a "shepherd" or facilitator who provides continuity, guidance, and experience to the Chapter's leaders and the Brothers at large.
Specifically, the Advisor should:
  • Meet on a regular basis with the Chapter's leaders (e.g., officers and committee chairs).
  • When possible, attend the formal or another meeting of the entire Chapter on a monthly basis.
  • Be available to advise and counsel any Brother in need of such.
  • Serve as a point of contact for the National and Regional Officers with regard to the Chapter's
  • Compliance with established Chapter Standards and fraternity and college policies.
  • Serve in any other capacity mutually agreed upon by the Advisor and Chapter.
