Approvals: This proposal has been reviewed and approved by ALIVE Principal Investigators as of ______(date). The Senior ALIVE Investigator identified as the primary contact and collaborator for this proposal is: ______.

Scope of project: Specimens or data provided by ALIVE are intended for the express purpose of performing the approved research project. Data received from ALIVE should be kept confined to the specific aims of the analysis proposed. Additional initiatives should be submitted to the PI’s along with completion of a new Concept Sheet or Data Analysis Request Form. These specimens and data must not be provided to other investigators or used for additional projects (or in classes for teaching purposes) without the written consent of an ALIVE Principal Investigator. If data analysis for the project has not been carried out at the ALIVE DataHouse (DH), the first author is responsible for sending the computer programs, final data sets and codebooks that directly relate to tables and figures in the manuscript to the DH. The programs and data should be clearly labeled, include a data dictionary, and should be sent along with the reprints to the DH.

Completion of Work: The expected date for completion of this analysis is ______. It is anticipated that a manuscript will be submitted based on this analysis within _____ months from completion of the analysis. Adjustments to this tentative schedule should be communicated promptly to the ALIVE PI’s. It is understood that if these deadlines are not met and no communication or agreement on a revised timeline is made, this project and accompanying data may be reassigned to another investigator.

Publications & Presentations: All abstracts and /or manuscripts MUST be submitted to the ALIVE Principal Investigators and /or ALIVE collaborating authors for review and approval before they are submitted to a conference or journal. Generally, an ALIVE co-author will be included in the project if there is a substantive contribution. Rare exceptions may occur (e.g., for data used for methodological development) but delineation of authorship should occur during project planning stages. Sufficient time for revision should be allowed before submission. Also, final revisions should be available to co-authors for review before resubmission. Lead authors should notify ALIVE Investigators of any manuscripts accepted for publication and promptly submit a copy of any published manuscripts to Rhonda Carroll (:) to maintain an archival record of work resulting from the study.

Acknowledgments: All publications and presentations of studies utilizing samples and/or data supplied by ALIVE should acknowledge both the contributions of samples and the collaboration itself; for example:

“Data presented in this manuscript were collected by the ALIVE Study which was funded by the National Institute of Drug Abuse through grants DA004334 and DA12568.”

Submission of this form and receipt of data indicates mutual acceptance of this agreement.
