Agreement for Postpartum Doula Services

We offer postpartum doula care for you and your newborn infant(s). Our services include the following:

  • Emotional support and guidance
  • Feeding/Breastfeeding support
  • Newborn care suggestions and guidance
  • Telephone support for any questions or concerns at no additional charge
  • Food preparation and light meals
  • Grocery shopping & local errands (cost to be added to invoice)
  • Light housekeeping (laundry, dishes, etc)
  • Sibling entertainment
  • Referrals & Resources should you need additional support in an area that we are unable to assist with

Billing

  • There is a 25% deposit due at the time of signing this contract. This is a non-refundable retainer applied to the last week of services. Should you cancel services at any point without rescheduling the contracted hours, the deposit will not be refunded. The services will be paid for on a weekly basis as laid out on the next page. If mother/partner decides to contract the doula for more hours after the completion of this contract, she may do so pending the doulas availability.

Funds:

For errands and grocery shopping, receipts will be given to you and amounts will be added to invoice for the week.

Cancellation of service:

Please notify us 24 hours in advance if you need to reschedule a day of service to avoid a $50.00 late-cancellation fee. This fee does not apply in the case of a true emergency or severe weather condition cancellations. We will notify you 24 hours in advance if we need to reschedule service for that day for any reason. Any services that are not rendered on the agreed upon date will be rescheduled as soon as possible.

We ask that you notify me within the first 48 hours after the birth of your baby(s) so we can arrange our schedule. At this point, we will notify you of any possible schedule conflicts. I require 48 hours notice prior to services beginning. (Unless otherwise agreed upon and noted below.)

Package Selection:

I/We have read this contract describing postpartum services. I/We have read all statements contained herein and I/we fully realize that I/we are signing a complete release and bar to any claims, which I/we have or believe I/we may have resulting from our contract for postpartum doula services.
