Title Form Instructions

Photo Plaque

On earning a first title*, each dog is entitled to a wooden photo title plaque provided by GRAC. Please include 5x7 photo of dog, or give to Beth or Judy by February 15. *Note: If you are a new member to GRAC, it is not required that this be your dog’s very first title; the first title since becoming a member will entitle you to a photo plaque as well. If you need a New Title photo plaque, check here: ______

8x10 Plaque

Dogs earning a high level title (MACH, PACH, PAX, RAE, UDX, OTCH, etc.) are entitled to an 8x10 plaque provided by GRAC. Second level of these titles can obtain shingles to add to their 8x10.

New Titles

Dogs earning titles through the MX, UD, RE will receive title Shingles if your dog already has a photo plaque. Please check box on your form whether or not you would like to receive these.

NOTE: Please complete and return this form to Beth even if you do not wish to receive plaques or shingles. GRAC would like to use our Awards Program to recognize the outstanding performances of all of our dogs in the past year! If you are not sure about what you are entitled to, please email Judy for clarification so you don’t miss out!
