AFSNCOA BOMB THREAT CHECKLIST

1. / Actions taken should be responsive to the emergency and taken seriously until proven otherwise.
2. / Utilize the Bomb Threat Form, AF Form 440 or the Bio/Chem Agent Threat Form, MAFB 48-1 respectively,located next to each phone, to record as much information as possible. Write down the communication word for word.
3. / Leave the telephone line open after the caller hangs up. Under some conditions, it may be possible to trace the call.
NOTE: From this point forward, no cell phone, radio or other electronic device should be utilized within the building as this could case detonation
4. / Notify AFSNCOA Command Section, as soon as possible either during or after the call is completed.
After all personnel are accounted for, notify the UCC/CATwith the status of all staff/faculty and students. UCC/CAT: Primary #416-1830—If UCC relocates: #416-2283; Maxwell-Gunter command post if contact cannot be made with the UCC/CAT: 953-7333
5. / Notify the Security Forces and the Fire Department as soon as possible via 911. (911 from landline; 953-9911 from cell phone)

6. / Designated instructors will notify all occupants to evacuate the building and provide the reason for the evacuation. The person who receives or detects the threat will use the first five instructors available to relay the threat to the areas specified below. One instructor will go through each squadron and notify each flight or office to evacuate the building and give the reason for the evacuation.
Instructor One will notify Alpha Squadron
Instructor Two will notify Bravo Squadron
Instructor Three will notify Charlie Squadron
Instructor Four will notify Historic Wing
Instructor Five will notify the AFSNCOA Leaderhip, GTNCOA CSS & EPMEIC
Instructors 1-4 will report to the established Entry Control Point (ECP) to brief the command staff of actions taken. Instructors 1-4 will then report to their assigned rally point at Fisk Park PT pad.
7. / Stay alert for any strange or unusual objects in your area.
8. / Personnel will use the fire evacuation routes (if possible) to evacuate the building and assemble at Fisk Park.In the event a 4000-foot cordon is established, the rally point will then be at theGunter Bowl.
9. / Upon evacuation, all electrically connected items will be left intact. Lights, switches, televisions, etc will be left on/off as applicable.
10. / Any person within the building who finds an actual or suspected bomb will leave the actual or suspected device in place and report findings to the UCC @ 416-1830. DO NOT TOUCH THE DEVICE!
11. / Squad Leaders or a designated instructor will conduct a sweep of the building to ensure all personnel are evacuated.
12. / Flights will form up by squadrons at the center of Fisk Park PT pad, beginning with Alpha Squadron closest to EHRI and ending with Charlie Squadron closest to perimeter fence. AFSNCOA staff/faculty will form in front and center of all student squads.
The class president will direct student squadron and flight leaders to conduct roll call and account for all their students. The squadron leaders will report their squadron’s accountability to the class president, who in turn, will report status to RP, Registrar or AFSNCOA EM accountability email account.
13. / Squadron Leaderswill account for all of their personnel and report to the command staff through RP.
14. / Director of Education and Director of Resource Programs will account for all students and staff and report this information up the chain of command.
15. / All personnel will remain at the Rally Point until released by the chain of command via Barnes Center/UCC.
