I am writing to you following the meeting of the investigating sub-committee of the Board of Studies/Graduate School Board in [location] at [time] on [date], and approval by the Standing Committee on Assessment of the process followed at that meeting.

I am writing to let you know the outcome of the sub-committee’s consideration of the material that constituted academic misconduct in the work you submitted for [module/assessment name]. The minutes of the meeting of the investigating sub-committee are enclosed.

The investigating sub-committee decided that academic misconduct had taken place.

The sub-committee will recommend to the Board of Studies/Graduate School Board that you receive an academic mark of XX for the work submitted. This mark is subject to final approval by the Board of Studies/Graduate School Board of [department or combined board name]. This mark is not a penalty.

The Board will write to advise you of the outcome of their consideration of your case.

The sub-committee will also recommend to the Board of Studies/Graduate School Board that Penalty Points of XX should be deducted from the overall mark on which your award is based. This recommendation may be subject to further consideration by the Special Cases Committee (for undergraduate students)/ the Board for Graduate Schools (for postgraduate students) depending on the level of penalty.

You must note that if you carry out any further academic misconduct the Board of Studies/Graduate School Board may recommend that your enrolment with the University be terminated.

This applies regardless of the size of the penalty you will incur in this case and even if the work in question does not contribute to the final award.

ANDIf this is an investigation of a second incidence of academic misconduct, normally

The sub-committee will also recommend to the Board of Studies/Graduate School Board that you should not be permitted to continue with your registration at the University of York, as this is the second incidence of academic misconduct that you have committed. I attach a copy of the form you signed on [date] following the previous investigation.

Confirmation of this recommendation is subject to consideration by the Special Cases Committee (for undergraduate students)/ the Board for Graduate Schools (for postgraduate students).

The Special Cases Committee will write to advise you when it will consider your case.
