ADVOCACY
QUALITY
ACCOUNTABILITY /
Supporting or recommending a certain cause or policy is known as advocacy. To fulfill your obligation as a patient advocate, you must provide the patient with the best possible care from all professionals, including yourself. To achieve the best possible outcome, you must evaluate the patient’s orthopaedic condition, general health including comorbidity and the various available clinical treatment options. To properly weigh the various clinical treatment options you must be familiar with Level I & II medical evidence from peer-reviewed published literature, as well as other scientific information on a given topic. Although knowledge alone does not guarantee a good outcome, it allows the surgeon to focus on the safety and efficacy of a given treatment option. Providinghigh quality care leads to high patient satisfaction.
Accountabilityis the fact or condition of being responsible. Surgeon accountability in providing quality care is extremely difficult to assess.
The concernspertaining to qualityare:
- What specific methodology and mechanism(s) should be used to assess the quality of care?
- Who should determine the criteria for acceptable quality ofcare?
- A committee of physicians and non-physiciansknowledgeable in the fieldcould establish the methodology and mechanism(s) for acceptable standards of patient care.
- Every quality measure ought to be risk adjusted based on pre-operative comorbidities, includingmethods and mechanism(s) which are necessary to report deviations.
- The committee should identify quality deviation(s), take corrective action whenever necessary, including the educational process required to improve the deviation.
In recent years, issues of ethical behavior,conflict of interest and deceptive marketing in the medical profession have entered the public forum because of widespread use of the following:
- Metal on metal hip replacement
- Ceramic on ceramic hip replacement
- MIS surgical techniques
- Navigation for use on all patients
In the end, the primary objective of achieving a good outcome and a high level of patient satisfaction is to provide the best and most compassionate care under the given circumstances. No surgeon should be penalized or held accountable for quality of care under difficult circumstances beyond the surgeon’scontrol. However, the old saying, “know thyself,” is particularly applicable here. As a surgeon, you must know and respect the parameters of your God-given abilities, medical training, and knowledge of the literature. Remember that you are the only one responsible for your conduct, clinical choices, and results.
