I, the above named Adopter, understand and agree to abide by the following conditions and restrictions in order to adopt the dog named above (hereinafter referred to as "the dog") from German Shepherd Rescue of Sacramento Valley (hereinafter referred to as "Rescue).
I will provide the dog with a loving and secure home, and protect the dog from neglect, abuse and cruelty. I will accept this dog into my family, and will provide it with proper food, fresh water, adequate exercise, and shelter at all times. I will not keep the dog tied up in my yard for any extended period of time. I will provide veterinary treatment as needed, including all vaccinations and tests to maintain the dog's good health. I will maintain the dog on heartworm preventative for the rest of its life. At no time will I allow the dog to ride unsecured in the bed of a truck. I will ensure that the dog wears a collar with at least one ID tag at all times and that the information on those tag(s) is current.
If for any reason I can no longer keep the dog, I will request assistance from Rescue to find the dog a new home. I will not sell or give away the dog—even to a family member or friend—or under any circumstance allow the dog to be admitted to an animal control facility, animal shelter, humane society or any such agency, regardless of circumstances. I will not sell or donate the dog to be used in research, vivisection, fighting, guard work, or any commercial activity. If the dog bites anyone, including a member of my family, I will not have the dog euthanized, but will rather contact Rescue to discuss how to proceed. Rescue retains the right to take back a dog who bites anyone.

I acknowledge that Rescue charges adoption fees for their dogs to cover veterinary and other expenses. I agree to pay to Rescue the fee that applies to the dog that I am adopting. In most cases, the adoption fee is $200 for purebreds, $175 for mix breeds, and $150 for dogs over 7 years of age. All fees include a six-month supply of heartworm preventative. Rescue has the discretion to charge more or less for a dog, depending on circumstances and the dog being adopted. I understand that Rescue will return my adoption fee if, within two weeks, I choose to return the dog to Rescue. If I choose to return the dog to Rescue after two weeks, Rescue is under no obligation to refund any of the adoption fee. (However, Rescue will generally extend the adoption fee refund period if, prior to two weeks from the date of adoption, the adopter requests more time to help the dog settle into its new home.)
Except for any adverse conditions noted, the dog is presumed to be in good health at the time of adoption. Rescue assumes no responsibility for any adverse health conditions which are not in evidence at the time of adoption. I understand that Rescue does not have the ability to guarantee the dog's good temperament or its good behavior once in my ownership. Rescue assumes no liability for any damage or injury to person or property that may result from this adoption, including injury from the dog biting a person. I agree to hold Rescue harmless in all regards to my adoption of the dog by this Contract.
I have read and understand this Contract in its entirety and agree to abide by all the conditions an restrictions imposed upon me by Rescue, which are designed to safeguard the health and well-being of the dog.
