DOCUMENTS TO BE SUBMITTED AT THE TIME OF ADMISSION

I) ADMISSION UNDER RTE

1) A Copy of the online registration form.

2) Acknowledgement slip, with Photograph of the child.

3) Original Date of Birth certificate for verification, along with a copy for record.

4) Latest Address proof in the name of the parent: - (issued within 6 months)

(Latest Residential / Nativity certificate, Latest Voter’s identity card, Latest telephone bill etc. - any 1)

a) Original BPL Certificate of father. (If applicable)

b) Income certificate from the Village officer. (For EWS candidates, if applicable)

c) Original OBC Non Creamy layer certificate for verification along with a copy for record. (From Tahsildar, if applicable)

d) Original SC/ST Certificate in the name of the child for verification along with a copy for record. (If applicable)

6) Self declaration by parents regarding distance from KV EZHIMALA to residence of the child.

7) Original Certificate showing the disability from Civil Surgeon (For Differently abled candidates) for verification and a copy for records.

II) FOR ADMISSION UNDER SC/ST CATEGORY

1) A Copy of the online registration form.

2) Acknowledgement slip, with Photograph of the child.

3) Original Date of Birth certificate for verification, along with a copy for record.

4) Latest Address proof in the name of the parent: - (issued within 6 months)

5) Original SC/ST Certificate in the name of the child for verification along with a copy for record.

6) Original Certificate showing the disability from Civil Surgeon (For Differently abled candidates) for verification and a copy for records.

III) ADMISSION UNDER CATEGORY – I & II (CENTRAL GOVT EMPLOYEES including EX- SERVICE MEN & AUTONOMOUS BODIES UNDER CENTRAL GOVT.)

1) A Copy of the online registration form.

2) Acknowledgement slip, with Photograph of the child.

3) Original Date of Birth certificate for verification, along with a copy for record.

4) Latest Address proof in the name of the parent: - (issued within 6 months)

(Latest Residential / Nativity certificate, Latest Voter’s identity card, Latest telephone bill etc. - any 1)

5) Original Caste Certificate in the name of the child for verification along with a copy for record. (If applicable)

6) Service certificate and Details of transfers from competent authority / Departmental Head (For service personnel)

7) Original Certificate showing the disability from Civil Surgeon (For Differently abled candidates) for verification and a copy for records.

IV) ADMISSION UNDER CATEGORY – III & IV (STATE GOVT EMPLOYEES & AUTONOMOUS BODIES UNDER STATE GOVT.)

1) A Copy of the online registration form.

2) Acknowledgement slip, with Photograph of the child.

3) Original Date of Birth certificate for verification, along with a copy for record.

4) Latest Address proof in the name of the parent: - (issued within 6 months)

(Latest Residential / Nativity certificate, Latest Voter’s identity card, Latest telephone bill etc. - any 1)

5) Original Caste Certificate in the name of the child for verification along with a copy for record. (If applicable)

6) Service certificate and Details of transfers from competent authority / Departmental Head (For service personnel)

7) Original Certificate showing the disability from Civil Surgeon (For Differently abled candidates) for verification and a copy for records.

V). ADMISSION UNDER SGC

1) Original Date of Birth certificate for verification, along with a copy for record.

2) Latest Address proof in the name of the parent: - (issued within 6 months)

(Latest Residential / Nativity certificate, Latest Voter’s identity card, Latest telephone bill etc. - any 1)

3) Original Caste Certificate in the name of the child for verification along with a copy for record. (If applicable)

4) Original affidavit showing Single Girl Child status from the NOTARY advocate.

5) Original Certificate showing the disability from Civil Surgeon (For Differently abled candidates) for verification and a copy for records.

NOTE:

v  Short listed students’ list for under admission under various categories will be uploaded on school website on evening of 22.03.2017.

v  Child and the parent must be present at the time of admission.

v  All the original documents corresponding to the information provided online should be produced at the time of admission.

Failure to produce the original documents will lead to Cancellation of admission.

v  Admission should be taken on the scheduled date(s) itself. Failure to do so may lead to cancellation of admission and chance will be given to the next deserving candidate.
