Michigan K-12 P.E. for Students with Disabilities (SP) EndorsementChecklist

1)You will need to have completedthe requirements listed below.

2)Official transcripts of any approved transfer work must be on file at WMU.

3)Report HONESTLY when applying with MOECS, all felony and misdemeanor convictions (regardless of how long ago). You will be required to provide a Judgment of Sentence or Register of Action for each offense, from the court to the WMU Teacher Certification Office. New information must be reportedto the address below within ten days of the offense.

Note: This ENDORSEMENT may only be added to an active teaching certificate. Please apply (using the MOECS system) for this ENDORSEMENT using the instructions below.

4)Must create MEIS account (Michigan Education Information System) if you haven’t done so already.You will need this account to apply for certification using the MOECS (Michigan Online Educator Certification System). Instructions below. Do not create a second account.

5)Apply using MOECS, Michigan Online Educator Certification System when you have completed #3 and #4 below. The WMU Teacher Certification Office will be notified that you have applied, and will contact you if additional information/documentation is needed. Instructions below.

Step 1: Create a Michigan Education Information System (MEIS) Account

Visit / and follow the links on the screen to create a MichiganEducation Information System (MEIS) account. MEIS will email you a user ID and passwordalong with the MEIS account number to the email address provided by you during MEISregistration.

Step 2: Register with MOECS

Once you have established a MEIS ID, please visit and on thelogin screen sign in with your MEIS user ID and password and follow the steps to self registerwith MOECS. On the registration page, you will be asked to provide your MEIS account number,which is included in the email that you received from MEIS.

Step 3: Apply for Certificate in MOECS

Once you have signed in with MOECS successfully, you will be asked to complete thedemographic information and once it is saved, you will see the links on the left navigation panel.Choose the link that is appropriate for you and follow the steps to apply for your certificate.

Step 4: University/College review/approval

After you have applied for the certificate, your application will be submitted to the institutionthat you identified in the application process via MOECS for review and approval. PLEASE MAKE SURE THE RECOMMENDING INSTITUTION HAS THE ITEMS LISTED IN THE CHECKLIST ABOVE ON FILE PRIOR TO APPLYING.

Step 5: Online Fee payment

Once your application is approved by the institution in MOECS, you will receive an email with alink for you to pay the fee online using credit/debit card or you can do so by logging into MOECSusing your user ID and password.

Step 6: Issuance of Certificate

Once the fee is paid, your application will be approved by the Michigan Department ofEducation, Office of Professional Preparation Services (OPPS) and the certificate will be printedand mailed to your address as stated in your demographic page within 7-14 business days.

I have completed 22 hours of approved Special (Adapted) Physical Education courses, with courses as listed below.
