Activity Report Data Collection Form

Purpose:To provide total annual activity hours in the activity subjects for our 7c, AB 204, and other grants per our reporting requirements.

General

Information:The Activity Report Data Collection Form (See Attachment A) provides our funding sources and CIL with information regarding the hours staff spends working in our communities in relation to our Service Program Goals. This hour data collection is part of the criteria for our community participation services requirements.

Procedure:1.This form is to be used only by the staff or volunteer (i.e. Senior Aides, Department of Employment and Training, Summer Youth, Interns, and etc.) who is collecting the data from the Activity/I&R Forms that the Service Providers use.

  1. To record the data needed the person collecting this data must have the completed Activity/I&R Forms.

  1. When filling the form out the data collector must identify the month and year of the data and the office from which the data is being collected.
  1. Under Activities (which is located middle/upper left side) the data collector will start recording the activity hours located on the Activity/I&R Forms located next to the date and time.
  1. The data collector will go through all the Activity/I&R Forms recording the hours in tally style recording it under the activity section.
  1. The data collector will then tally the same hours marked under each activity subject category. This section is located at middle/bottom, portion left side of the Activity Report Data Collection Form.
  1. The data collector will make sure that each activity subject that is marked has an hour tally marked next to it same as what is marked under Date and Time on the Activity/I&R Form. In other words, the activity section would have the hours from the date and time marked. Also, two activity subjects would have the same hours recorded.
  1. Once all the hour tallies are completed the Activity section is totaled and then each Activity subject is totaled under the Activity Subject section. The totals are recorded under total hours column on the right side of the Activity Report form.
  1. These forms are all then turned into the Director of Services to be used for the Grant Reports.
