Administering the Annual Strategies for Success Online

The following are step-by-step instructions for administering the Annual Strategies for Success (ASFS) on-line. REMEMBER, you may administer the ASFS only after you have submitted a data collection protocol and received approval from PIRE. Collecting these data on-line presents unique challenges that need to be carefully thought through prior to any data collection taking place.

Option A:If you choose to administer the survey in a school with wireless internet accessor with a computer lab linked the internet, please follow these steps:

1)Each computer or tablet should be logged onto the correct survey (either middle school or high school) prior to beginning data collection. These are the direct links to each survey:

Under some conditions, it may be appropriate to use a QR-code for youth who wish to use the survey on their own mobile phones or tablets (assuming they have QR reader apps). You can print this code onto cards that you hand out to participants. Do not have participants download the Qualtrics app to their phone, rather this QR-code will take the participant directly to the survey.

2)Review the Student Assent documentation with the students and ask them to sign and turn in the form. Any student not wishing to complete the survey needs to let the test taker know.

3)On the first page of the on-line survey, you must log in to the survey by entering your SITE ID, School ID, and School Name. This information is required. You can enter this information prior to giving the tablet or computer to the student, or provide the information to the student and have him/her enter the information directly. Only by entering this information, can you access the survey.

4)Ask the students to select whether they want to take the survey in English or in Spanish by selecting the language in the upper right corner of the document.

5)Read the consent information through with the students and ask if there are any questions. If questions, please answer them as precisely as possible.

6)Ask students to select whether they want to take the survey or not and click the arrow on the bottom on the page and begin.

7)Remind students not to talk or discuss the survey with each other while completing it.

8)Provide the students instructions on what to do once they have completed the survey and submitted it. You may want them to raise their hands so you can retrieve a tablet, or you might wish for them to stay quietly in their seats so they do not interrupt the other participants.

9)Provide every student with the take-home information in both English and Spanish.

10)Thank the students for their attention and time.

Option B:If you are administering the survey using tablets without internet access, please use the following instructions:

1)You must make sure the Qualtrics App is downloaded on all tablets. (The app is already installed in all OSAP iPads. iPad screen logon is 000000 – next we will explain how to work with this)

a)Find “Qualtrics” app on your IOS or Android device

b)Download app

c)To download survey, when the app opens, you will be told to enter a

Username: & aPassword: SFS2017*

d)Download the survey (one for high school and one for middle school)

e)When survey requires updating, you will be informed to “refresh”

If you are working with the App already installed on a device (such as the iPads used for the NMCS, screen password 000000),do the following after you are connected to wifi

a)Log out of the app (hit the gear button on the upper right hand screen and press the green log out button).

b)To download survey, when the app opens, you will be told to enter a

Username: & a Password: SFS2017*

c)Download the survey you want (High school or Middle school)

d)When survey requires updating, you will be informed to “refresh”

e)You should not have to go to the App store to download the App again. If you need the apple ID for the iPad please communicate with Coop Consulting.

7)Remind students not to talk or discuss the survey with each other while completing it.

8)Once the students have completed the survey and closed it, collect the tablets or computers (if appropriate).

9)Provide every student with the take-home information in both English and Spanish (print one side Spanish and the other English).

10)Thank the students for their attention and time.

Every day you collect data, do the following (not required after every response on a device just at the end of each day).

11)When you have access to the internet, upload all surveys and close out of Qualtrics.

12)When you have access to the internet, upload all surveys. You should see the number of pending responses reduce and the number of uploaded responses increase.

13)Finally, ONCE YOU HAVE UPLOADED SURVEYS, remove pending responses from the survey. Closing the app alone is not sufficient. Using the manage/settings/gear logo, select remove responses (Delete All Responses: This will permanently remove all pending responses from this survey. Select Delete. This will insure that these surveys are not continuously uploaded in the future.

14)Next, hit the “X” in the upper right hand corner of the survey screen. You are now in the main App menu. Go to the gear button and select log out. Then you are asked to delete X number of responses (you’ve already uploaded so it’s ok). Select delete and you are back to the setting screen where you have to enter user name and password.

When you initiate data collection again, you must enter the name and password again.
