Four Week Residential Treatment ProgramInformation Sheet

Thank you for your interest in Action North Recovery Centre’s Residential Treatment Program located in the town of High Level, Alberta. Any information provided is confidential. Once your completed application and medical forms are received, you or your referral will be contacted for an intake interview. This will determine your acceptance into our program

Admission Process

Thoroughly complete all pages of the application form.

Ensure to sign and date the application.

The Medical Form must be completed and signed by a physician/registered nurse.

Mail, fax or drop off admission form.

ANRC requires a completed admission form before an intake date can be made.

Confirmation of payment must also be received prior to the admission date.

You must have five (5) days clean from alcohol, drugs and gambling to participate in the program, including those that are not a problem for you. Detox may be required.

A return bus ticket is required for those arriving via Northern Express bus line.

Program Fees

  • For residents of Alberta, a room and board fee of $40 per day is charged
  • Out of province residence fee is $150 per day

This fee is to be paid prior to or upon admission. If a funding agency is providing payment then a completed payment confirmation form is required prior to admission.

This fee does not cover transportation and personal expenses.

Cash, cheque, or money orders are accepted payment methods. Please note that we do not accept payment by credit card.

Do not schedule any appointments (personal, legal, dental, medical) while in treatment. You will need to focus on your program during your time here and appointments will be made on an emergency basis only.

Checklist of Suggested Items to Bring:

If you smoke, bring cigarettes since they are not sold on the premises

Travel coffee mug (with lid)

Casual clothing; loose fitting and comfortable

Weather appropriate clothing: winter boots, jacket, toque and mitts

Indoor shoes or slippers

Hygiene items such as; shaving supplies, shampoo, conditioner, body wash, bar of soap, toothpaste, toothbrush, deodorant, brush/comb, hair products, feminine products......

2” binder to keep handouts/workbooks if desired

Notebook and pen (journaling/notes)

Alarm clock

Housecoat & pyjamas

Mp3 player or Discman with headphones *NOTE: No music devices with cameras

Spending money (in small bills) & change for payphone/calling cards or for our vending machine.

Bring your current Alberta Health Care card and Alberta Works Benefit card

Hobby items: beadwork, crochet/sewing, word searches or books.

DO NOT BRING:

Large sums of money

Knives or other weapons

Unsealed, unlabeled or mixed medication (prescription or non-prescription)

Office or school work

Pornography; movies, magazines, posters

Laptops, iPads, iPods, or other electronic devices

Camera

Alcohol based mouthwash

Energy drinks, bars or powders

Inappropriate clothing (revealing shirts/shorts)

Expensive jewelry

***NOTE: We do not have a bank machine on premises. Make sure all banking needs are done before arrival & ANRC is not responsible for lost or stolen items or for items left behind.

12 Step Meetings

  • Action North is a 12 step based treatment program. Alcoholics Anonymous and Narcotics Anonymous Meetings are a part of our program and are mandatory.
  • All 12 Step meetings are open meetings held at the Centre during the following times
  • A. A meetings –Tuesday, Thursday and Friday at 7pm
  • N. A meetings – Monday, Wednesdays at 7pm
  • During the Four Week program, clients will work through A. A steps 1-3
  • Big Book studies are done on a weekly basis and Big Books are available to use while in the program

Medication

  • Please bring all prescribed medications with you when you come to treatment.
  • Ensure that there is a one (1) month’s supply of medication brought with you.
  • Multiple medications must be in bubble pack form.
  • Medication will only be administered according to the prescription label.
  • Medication not in proper packaging/bottles will not be administered during your stay.
  • All non-prescription medications/vitamins must be properly sealed or they will not be administered.
  • Non-approved medications will not be administered and stored away until discharged.

Clothing

  • Bring comfortable clothing to wear during your stay here.
  • In addition, many clients may prefer to have a nice outfit to wear to their graduation.
  • Comfortable indoor shoes or slippers and proper outdoor wear (jacket, gloves, hat, shoes, boots) are also required, as we go for daily walks.
  • Swimming trunks/suits if you wish to attend the pool in town during weekend recreation.

Accommodation

  • ANRC provides shared accommodation and there are no individual rooms.
  • Each room has a private bathroom for occupants.
  • Due to dresser space, please do not pack large amounts of clothing. We have laundry facilities on site.
  • Rooms are to be tidied up daily and beds are to be made each morning.

Telephones and Messages

  • ANRC has a pay phone for personal client use, and can be utilized daily within the phone hours posted (12pm-1:30pm, 4:30pm-7pm, 8pm-11pm)
  • Due to our confidentiality policy, we do not respond to any personal or telephone inquiries about your participation in treatment.
  • Individuals can leave messages and they will be placed at the front desk for you.
  • Cell phones are no permitted and will be locked away until discharge.

Mail

  • Postage can be purchased from the front reception.
  • Mail is posted in the afternoons, usually before 3pm.

Laundry

  • Washers and dryers are available for use during posted times (12pm-9pm) as long as it does not interfere with session/group.
  • Laundry soap and dryer sheets are provided at the front desk and an iron is available as well.

Recreation

  • All group members are required to participate in leisure activity, walks, and energizers as part of the recovery program..
  • You may need to bring some money if you choose to participate in leisure options that are not funded (pool, town events)
  • Exercise equipment is available for your use at the Centre.
  • Items available for recreation use include: footballs, baseball gloves & bats, skates, bicycles, badminton rackets, croquet set, horse shoes and volleyballs.

Lounge Area

  • A guitar, pool table, dart board, assorted books, puzzles, crib boards, cards, dice, and board games are available.

Client Chores

  • Chores are to be completed on a daily basis.
  • There is a rotation list that is posed every Tuesday, and all clients work together in doing their part.
  • Duties, house rules and expectations will be explained during orientation
  • In addition to regular daily chores a major clean-up is also conducted every Friday, a separate chore list will be posted for this.
  • Clients are expected to maintain cleanliness of rooms.

Visitors

  • Visits should not interfere with program time.
  • Visitors are welcome during the weekends during the following hours:

Sundays6pm-8pm

Funding

  • It is your responsibility to arrange cost of treatment with funding sources available to you, such as Alberta Works, OHI, etc. prior to arriving.

When you arriveviaNorthern Express Bus:

  • The bus arrives at the Four Winds Hotel.
  • Contact us when nearing High Level, or if you do not have a cell phone, then call us once you have arrived.
  • A staff member will go pick you up.

When you get dropped off or via personal vehicle:

  • Come on in and introduce yourself!
  • Staff will be available to greet you and check you into your shared room.

Please note that a thorough baggage check will be completed upon arrival and we appreciate your help with this.

We will also gather items that are to be stored at the front desk: cell phones, items containing alcohol, and medications/vitamins.
