POST: GP Carer Awareness Adviser (CAA) South West Surrey

RESPONSIBLE TO: Senior GP Carer Awareness Adviser

JOB PURPOSE:

To work in partnership with the senior CAA in delivering the GP Carer Awareness service within South West Surrey ( Guildford, Waverley, Surrey Heath and Farnham) to include the development and planning of organisational activities, monitoring and evaluation of the project, within the overall aims and objectives of Action for Carers Surrey.

SUPERVISORY RESPONSIBILITES:

None

HOURS: Part Time 20 hours per week, including some occasional evening and weekend work, planned in agreement with senior CAA, for which time off in lieu will be accorded.

SALARY: NJC Scale Point 25 -27 (plus 1%)

OFFICE BASE: BURPHAM, GUILDFORD

MAIN DUTIES AND RESPONSIBILITIES:

1.  To work in partnership with the senior CAA in the development and implementation of the GP Carer Awareness project within South West Surrey to ensure in conjunction with the senior CAA that all activities of the GP CA project meet current legislative requirements, good practice and ACS policies and procedures

2.  To attend and contribute to monthly CAA team meetings

3.  In liaison with Senior CAA, to report to, and attend as required, the South West Surrey forum and other meetings as required.

4.  To work in partnership with primary health care teams, hospital departments, pharmacists, faith sector and the voluntary sector to promote the GP Carer Awareness service in South West Surrey and to build good working relationships with local Clinical Commissioning Group contacts.

5.  To work closely with local Carers Support services in South West Surrey, including developing existing links with GP surgeries in South West Surrey , and to liaise with the Carers Support schemes on all promotional activities .

6.  To enable GP practices to identify carers - both new patients and existing patients - by promoting Carers registration within each surgery, using the Royal College of General Practitioners/ Carers Trust good practice guidance and by promotion of the use of the toolkit.

7.  To increase GP and other practice staff awareness of carers and the possible impact to their health due to their caring role and enable them to signpost carers to the most appropriate place for further support.

8.  To provide up to date, relevant and accessible information regarding carers within each surgery in South West Surrey including Carers Information folders, supplies of the joint Surrey County Council & NHS carers’ information packs and provision and maintenance of carers’ notice boards within surgeries

9.  To encourage carer friendly practices within surgeries and health centres

10. To recruit a carers champion in each surgery in South West Surrey and encourage them to contribute to the carers champion network newsletter.

11. To maintain regular contact with all surgeries in South West Surrey and to organise an annual Carers Awareness event in each surgery.

12. To contribute to the design and update of promotional materials for the GP CA project including the GP Carers Awareness service website and promotional leaflets, ensuring carers’ views are reflected.

13. To undertake presentations or training sessions on the work of the GP CA service and the RCGP dvd ‘Supporting Carers in General Practice’ and otherwise promote the service within South West Surrey and Surrey wide in liaison with Senior CAA.

14. To produce a quarterly report and statistical analysis on the South West Surrey GP CA service to the Senior CAA for inclusion as part of the GP CA service report to the ACS Board, and any other such reports as may be requested.

15. To undertake other duties as may reasonably be agreed with the senior CAA.
