About this Checklist

This checklist is completed by the Workplace Assessor following the initial interview or discussion with the student and Workplace Supervisor. It ensures that important information regarding the Workplace Assessment and Work Placement processes is communicated to both the student and the Workplace Supervisor by the Workplace Assessor.

Submission Instructions

The Workplace Assessor should complete this Microsoft Word document electronically. Once this document is completed, it should be submitted by the Workplace Assessor through the Initial Interview Checklist upload link in OpenSpace.
