This is to document confirmation of your notification that the above named student is a Special Education Student in your (stated below) Regular Education Classroom and to confirm that you have received a copy of the Snapshot IEP for Regular Education Teachers. This student has a formal IEP, which is available for you to review, on file with the Special Education Casemanger. In the formal IEP document you would find a complete account of this student's special education information.
To assist you with the needs and instruction of this student, you are being given a copy of the Snapshot IEP. This document contains the information that would be most pertinent to your classroom.
Please remember that this information is CONFIDENTIAL and should be kept in a locked location. Only you and the paraeducator assigned to the class in which this student participates should have access to this information. As the IEP Casemanager, I am available to answer questions and assist you with IEP related concerns. These concerns can be discussed during a co-plan time or feel free to contact me at another time.
The purpose of this form is to document that the appropriate information has been made available to all staff directly involved in meeting the educational needs of the above named student. Not all staff listed below will receive a copy of the Snapshot IEP. Federal SPED Guidelines require that all components of an IEP also be implemented by regular education staff.
