Accident / Incident Reporting to the Barbados Ships’ Registry

This report is to be completed by an official of BMSR on the occasion of any of the following occurrences:

Sinking, Grounding, Fire, Collision or Accident resulting in Loss of Life, Personal Injury or Pollution

Interference with the vessel in port /anchor or influenced while underway by unauthorised persons having unlawful intentions.

In cases where the incident has not resulted in any of the above this report may be completed by an Officer of the Company which has responsibility for the operation of the Barbados registered vessel.

It such cases it is a requirement that owners arrange for this form to be completed by a company representative having appropriate experience and faxed/emailed to BMSR within 5 days of the accident occurring. If such a person is not available to the company then a BMSR Nautical Inspector shall be appointed to this task by ship owner.

The assessment carried out by BMSR of the Accident Report together with a copy of the Accident Report will be forwarded to the Ship Owner and in the case where the accident has resulted on loss of life and/or pollution to the IMO.

Introduction

The purpose of this report is to document as soon as possible after the incident the main factors existing at the time, which may or may not have had an influence on the resulting incident.

The report has a format that can be utilised for the reporting of all types of occurrences by the leaving out of those sections that are not applicable.

Any Official Enquiry found necessary will use this Accident Report as its basis when reaching a decision regarding the prevailing influences and both their combined and individual effects upon the ship, pre-incident, during the incident and post incident.

GUIDANCE FOR COMPLETING BMSR ACCIDENT/INCIDENT REPORT FOLLOWING A DEATH ONBOARD

Please comply with the following actions and provide the information requested to the relevant Parties as promptly as possible.

Ship’s location at time of accident: confirm if national or international waters.

ETA port and country: confirm if waters national or international.

Request for local assistance: police to meet ship on arrival to take statements.

Depending on police report; confirm jurisdiction for any prosecution.

Provide opportunity for Police to interview passengers and crew,

Police should interview last person to see deceased alive.

The Operators Representative should establish from interviews following points and include this information in the BMSR Accident/Incident Report.

  1. Time and place deceased last sighted.
  2. State of deceased at the time of last sighting both mental and physical.
  3. Was there cause for the deceased to be at the location where the accident occurred
  4. Are there any contributing factors to the accident
  • Was the ship rolling/pitching?
  • Was there an obstruction to footing?
  • Was the deck wet or slippery?
  • Was there a lack of hand holds?
  • Was appropriate personal safety clothing and equipment being used at the time of the accident or death?
  • Was the deceased under prescribed medication?
  • Are there any facts relating to this death that are suspicious?

The Operator’s Representative is to provide a list of the deceased’s property to the local authorities and BMSR.

BMSR Accident/Incident Report is to be emailed to the Administration at the company’s earliest opportunity together with a copy of the Coroner’s Report
