> > Always triple check name on label vs name on tube

Always use the original tube when possible. If not, place in original tube rack with accession #.

ANY TEST NOT LISTED HERE MAY REMAIN IN ORIGINAL TUBE OR CONTAINER. RECENT CHANGES ARE IN BLUETYPE

Only labelswith identical letter ID codes AND accession numbers may be together on one tube.

KX: Chemistry/HDL “Olympus AU5231” - Must be in 10 mL Corvac/SST or plastic 10 mL PAML tube.

Do not transfer CMPA, BMPA, EP, RENALA, CO2 from 5 or 7ml tubes to 10ml.Rubberband small tube to 10ml tube.

SPECIAL NOTES

BB1 : Cells Note: ABO/Rh-Cells may be from black, red, or lav top tube.

BB : Serum - ABO/RH (1.0), RPR (0.5), MABS (1.0), etc.

C_ : Glucose (0.5) - Serum acceptable, but use gray top if one is available. If so, centrifuge sample, split, and write “PLASMA” on label. Label timed specimens carefully. (Marked in rack with colored tape)

SC/SC1:Serum label and cells label.

SHDI: ICA (ionized Ca+) – must remain capped in original vacutainer to maintain stability.

UG1 : Placed on small aliquot tube and rubber banded to original sample if submitted in brown bottle or urine container.

TEMPERATURE CODES

RT:Room temperature specimens - DO NOT REFRIGERATE! File in proper room temperature rack.

FZ:Must always be in plastic tube. FRZ samples should be in coded frozen box, but ALWAYS CHECK THE UNCODED FROZEN BOX IF NO FROZEN RECEIVED AT LABELING.

FG:Refrigerated samples.

HE:CBC2– If a SED is also ordered place SED label on a separate tube & band to CBC tube.

HS : SED - If CBC on same patient, rubberband the two tubes together.

HU:UA/UAX/UAM (2.0)OSMUR/OSMUQ (1ml) - Any container-type acceptable; ensure tight lid and no leakage.

HE:PT/PTT - Light blue top tube. May also be plasma or frozen plasma. Line up barcode.

PTT should be on ice, if not, always check coding refrigerator and uncoded frozen box.

XXXX:Extra sample. Mark with XXX. Give to a Troubleshooter with requisition immediately.

XX:Also TIQ filing code. Must be handled by Troubleshooter only.

SEND-OUTS : Always check test and temperature requirements!

Urine:Must be in brown aliquot bottle, preserved as needed, with top parafilm-sealed.

SH_:SHMC samples. Use one tube per test label, unless sample is small.

X_:All other clinical reference labs.

REMEMBER: Use your colors on all labels and # on all requisitions
