Foster Care Roster

Report Background The Foster Care Roster report provides a list of children in the care and custody of the county Departments of Social Service (DSS)

Report Purpose Provides Local Districts with a detailed list of children with a Child Status Code of 04 (active) or 06 (absent) in CCRS.

Target Users Managers and supervisors from Local Districts and Regional Offices

Classification Operational report

Prompts Upon opening the report, users are asked to choose whether to run the report for a District or Region. The following report prompt then appears:

·  County Name/Region Name: Select the name of the County or Region for which you want to run the report. Local Districts are listed alphabetically by name with ACS under “New York City”.

Data Item / Data Definition /
As Of Date / The latest refresh date of the OCFS Data Warehouse – that is, the last time data was retrieved from CCRS. Data for this report is refreshed each week.
County / The name of the local social services district currently controlling the child's case.
In/Out Indicator / Text indicating where the child is placed. Values include:
·  In County Placement
·  Out of County Placement
·  Out of State Placement
Agency ID / Unique 3-digit identification number assigned by NYS to the agency that most recently placed the child.
Agency Name / Name of the agency that most recently placed the child.
Current Facility ID / Unique identification number assigned by NYS to the facility where the child is placed. It is also known as the Vendor ID.
Facility Name / Name of the facility or foster parent where the child is placed.
Facility Type Description / This is the Level of Care (LOC) provided by the facility where the child is placed. Some values include:
·  Adoptive Home
·  Institution
·  Group Residence
·  Group Home
·  Agency Boarding Home
·  Certified Foster Home
·  Approved Relative Foster Home
·  Foster/Adoptive Home
Facility Status Code / The two-digit code corresponding to the operational or license status of the facility where the child is placed. Values include:
·  01 (Active)
·  02 (Inactive)
Address County / The 3-digit county code for the physical location of the facility where the child is placed.
Address Line 1 / The street address for the physical location of the facility where the child is placed.
Address City / The city name for the physical location of the facility where the child is placed.
Address State / The 2-digit state abbreviation for the physical location of the facility where the child is placed.
Address Zip Code / The zip code for the physical location of the facility where the child is placed.
Phone Number / The phone number for the facility where the child is placed.
Contact Name / The name of the primary contact person for the facility where the child is placed.
Resource ID / A unique 8-digit number, which identifies the resource associated with the facility where the child is placed.
Case ID / The district-assigned case number that uniquely identifies the case within the district or responsible state agency.
Case Name / The family name by which the child welfare case is identified.
CIN ID / Client Identification Number, which is a unique identifier for the child that is generated from the CCRS application.
Child Name / The last name, first name, and middle initial of the child.
CCRS Child Status Text / The status of the foster child in the CCRS case. Values include:
·  In 24 Hour Care
·  Absent
Gender Text / The gender of the child.
Ethnicity Type / A description of the child’s race. Values include:
·  American Indian or Alaskan Native
·  Asian
·  Black or African American
·  Native Hawaiian or Other Pacific Islander
·  White
·  Unknown
·  Other
Hispanic/Latino Indicator / Identifies whether the child is of Hispanic/Latino origin or not.
Current Age Number / The child’s current age (as of the data refresh date) in years, based on the date of birth. An age of zero indicates the child is less then 12 months old.
