ABSTRACT: ADHCC 2017 awards program information and application now available.

In 2017, the ADHCC will conduct its 17th annual awards program. Designed to recognize outstanding individuals and organizational contributions to the field of adult day health care, the awards also enhance the image of adult day health care within the long-term care continuum.

Similar to last year, there will be two award categories in 2017: the Christine M. Fitzpatrick Advocacy Award and the Outstanding Program of the Year Award. Any ADHCC member may nominate a colleague or herself/himself for the Christine M. Fitzpatrick Advocacy Award or his/her own program or another program for the Outstanding Program of the Year Award.

The two awards will be presented at the annual conference awards luncheon on Thurs., April 20, 2017 at the Embassy Suites in Saratoga Springs, NY.

If you are interested in nominating an individual or program for an award, please review the description attached and return an application to Meg Everett by close of business on Fri., February 3rd.

Adult Day Health Care Council’s annual awards recognize the very best of New York’s adult day health care programs and the people who contribute their time, energy and professionalism to the field of adult day health care. Please help us identify those worthy of this special recognition.

 Outstanding Program of the Year Award

Christine M. Fitzpatrick Advocacy Award

This award honors Christine Fitzpatrick, past executive director of the Adult Day Health Care Council, for her enduring, passionate advocacy for adult day health care. This award is given to an active ADHCC member who has demonstrated exceptional advocacy and leadership on behalf of their registrants, families, ADHC program and staff. This award is given to an individual who is committed to ADHC and encourages others to advocate for the elderly, disabled or chronically ill. Although important, advocacy is not limited to elected officials. This award may be given to the person who fervently advocates for registrants in care plan meetings, with managed care plans or physicians.

Outstanding Program of the Year Award

This award will be granted to the ADHC program that demonstrates creativity, teamwork, and determination to overcome challenges in a time of tremendous change. An outstanding program exists not because of one person but because of the many contributions made by the entire interdisciplinary staff. The program that improves the quality of service to registrants and utilizes creative problem-solving are the types of initiatives this award celebrates. Finally, the outstanding program is an active member of the ADHCC and is seen as a role model by ADHCC members on a statewide or regional level.

Please note: We will remove the name of the program and other identifying information for the award selection committee. Nominations will be judged on the merits of the program; the name of the ADHC program is not divulged to the awards committee until after the judging.
