THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that the narrative reason for her separation be changed on her report of separation (DD Form 214).
2. The applicant states, in effect, that she was actually discharged for “Other Designated Physical or Mental Conditions” under the provisions of Army Regulation 635-200, paragraph 5-17. However, her DD Form 214 indicates that she was discharged under the provisions of Army Regulation 635-200, paragraph 5-13 for a Personality Disorder.
3. The applicant provides a copy of her DD Form 214 and a memorandum from a clinical psychologist with the diagnosis that served as the basis for her discharge.
CONSIDERATION OF EVIDENCE:
1. She enlisted in Mountain View, California, on 21 October 2003 for a period of 5 years and training as a journalist. She completed her basic training at Fort Jackson, South Carolina and was transferred to Fort Meade, Maryland, to undergo her advanced individual training (AIT) as a journalist.
2. On 24 June and 27 July 2004, she was diagnosed as having a major depressive disorder by medical officials at FortMeade.
3. On 3 August 2004, the applicant’s commander initiated action to separate her from the service under the provisions of Army Regulation 635-200, paragraph 5-17, because of other designated physical or mental conditions.
4. After consulting with counsel, the applicant waived her rights and elected not to submit a statement in her own behalf.
5. The appropriate authority approved the recommendation for discharge on 11August 2004 and directed that she be furnished an Honorable Discharge Certificate.
6. Accordingly, she was honorably discharged on 16 August 2004. However, the DD Form 214 issued at the time of her discharge indicates that she was discharged under the provisions of Army Regulation 635-200, paragraph 5-13 for a personality disorder.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record clearly shows that the applicant was approved for discharge under the provisions of Army Regulation 635-200, paragraph 5-17, for “Other Designated Physical or Mental Conditions.”
2. However, her DD Form 214 was incorrectly prepared to reflect that she was discharged under the provisions of Army Regulation 635-200, paragraph 5-13 for a “Personality Disorder.”
3. Accordingly, it would be in the interest of justice to void her current DD Form 214 and issue her a new DD Form 214 showing that she was discharged under the provisions of Army Regulation 635-200, paragraph 5-17 for “Other Designated Physical or Mental Conditions.”

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by voiding her current DD Form 214 dated 16 August 2004 and issuing her a new DD Form 214 showing that she was honorably discharged under the provisions of Army Regulation 635-200, paragraph 5-17 for “Other Designated Physical or Mental Conditions.”
