THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show award of the Good Conduct Medal, the Army Achievement Medal with First Oak Leaf Cluster, and the Kuwait Liberation Medal.

2. The applicant states that she was in the process of reenlisting and noticed that the awards were not listed on her DD Form 214.

3. The applicant provides a letter dated 18 January 2005, a copy of her DD Form 214, and a copy of a certificate awarding her the Good Conduct Medal, and the Army Achievement Medal.

CONSIDERATION OF EVIDENCE:

1. The applicant is requesting correction of an alleged error which occurred on 12 December 1991, the date of her separation from active duty. The application submitted in this case is dated17 January 2005.

2. The applicant's DD Form 214 with a date of 12 December 1991 shows in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) that she was awarded the Army Service Ribbon, National Defense Service Medal, Army Lapel Button, the Southwest Asia Service Medal with two Bronze Service Stars, the NCO Professional Development Ribbon, the Overseas Service Ribbon, the Army Achievement Medal, and the Army Aviator Badge. There is no record showing she was awarded the Army Aviator Badge.

3. Headquarters, 2d Infantry Division Permanent Orders 105-16 dated 26 May 1988; show that the applicant was awarded the Army Achievement Medal (First Oak Leaf Cluster) for the period 27 July 1987 to 30 July 1988 for meritorious service.

4. The applicant's DD Form 214 dated 27 June 1988, shows the applicant served a period of active service from 1 October 1985 through 27 June 1988. Item 13 of that form shows she was awarded the Army Achievement Medal, the Good Conduct Medal, the Army Service Ribbon, the Overseas Service Ribbon, the Army Lapel Button, and the Air Assault Badge.

5. Military records show that the applicant served in Southwest Asia (SWA) from 1 October 1990 through 31 January 1991.

6. Army Regulation 635-5 (Separation Documents) provides that item 13 of the DD Form 214 will reflect decorations, medals, badges, citations and campaign ribbons awarded or authorized for all periods of service. Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214.

7. The Kuwait Liberation Medal, awarded by the Kingdom of Saudi Arabia (KLM-SA), was approved on 3 January 1992 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 17January 1991 and 28 February 1991.

8. The Kuwait Liberation Medal, awarded by the Government of Kuwait (KLM-K), was approved on 9 November 1995 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 2August 1990 and 31 August 1993.

9. The applicant is currently serving on active duty in the Regular Army in the grade of first lieutenant.

DISCUSSION AND CONCLUSIONS:

1. The evidence of record shows that the applicant was awarded the Good Conduct Medal for the period 1 October 1985 through 27 June 1988. Therefore, her DD Form 214 for the period ending 12 December 1991 should be amended to show the Good Conduct Medal.

2. The evidence of record shows the applicant was awarded a second award of the Army Achievement Medal. Therefore, her DD Form 214 for the period ending 12 December 1991 should be amended to show the Army Achievement Medal (First Oak Leaf Cluster).

3. The evidence of record shows the applicant was awarded the Air Assault Badge and not the Army Aviator Badge. Therefore, the Army Aviator Badge should be removed and the Air Assault Badge added to her DD Form 214 for the period ending 12 December 1991.

4. The evidence shows the applicant served in SWA from 1 October 1990 through 31 January 1991. As a result, she is entitled to the Kuwait Liberation Medal (KLM-SA) and Kuwait Liberation Medal (KLM-K). Therefore, it would be appropriate to add these awards to her record at this time.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

a. deleting the Army Achievement Medal and the Army Aviator Badge from her DD Form 214 with the ending period of 12 December 1991; and

b. adding the Good Conduct Medal, the Army Achievement Medal (First Oak Leaf Cluster),the Air Assault Badge,the Kuwait Liberation Medal (KLM-SA), and the Kuwait Liberation Medal (KLM-K) to her DD Form 214 with the ending period of 12 December 1991.
