REQUIREMENTS-CODE OF CONDUCT-DRESS CODE

UNIVERSAL STUDIOS SENIOR GRAD NITE HOLLYWOOD BASH™

Please note: All Students/Chaperones must be incompliance with Verdugo Hills High School PTSA,, Brooks Transportation and Universal Studio™ Requirements/Code of Conduct/Dress Code. Failure to adhere to any of these Requirements-Codes of Conduct-Dress Code may result in delayed admission, denial of admission or expulsion from the park. If participant is sent home it will be at his/her own expense without refund and parent or legal guardian will be notified. The participant, along with the parent/guardian will arrange for transportation home at their own expense.

VHHS PTSA REQUIREMENTS:

·  Permission Form filled out, Requirement Form signed/attached to Permission slip and payment made by April 17th 2017. No exceptions.

·  Participants must have a valid California or High School ID on their person when attending event.

·  Participant’s bags and self are subject to search prior to entering bus.

·  Participants are responsible for being on time for Check-In and Departure. Any participant excessively late (more than 10 minutes) to arrival or departure are subject to being left and it will be their responsibility to arrange transportation at their own expense.

Leave at home:

·  Backpacks, except the ones provided by VHHS PTSA at the time of the event. (PTSA will have a room at school for you to leave your backpack overnight).

·  Shoes with skate wheels, skateboards and scooters.

·  Medications, prescription or over the counter (unless approved ahead of time and a form is filled out).

Dress code: All clothing must be in good condition and appropriate for anticipated weather. *Refer to Universal Dress Codes on attire.

Code of conduct: Excessive rowdiness, fighting, damaging property, foul language, and any illegal act(s) including but not limited to stealing, shoplifting, selling/sharing/using drugs, will not be accepted and could lead to immediate removal from park and/or local police intervention. Participants are expected to act in a socially acceptable and family friendly manner.

Event:

·  Participants must participate via VHHS PTSA (No will call ticket pick ups will be available. Annual passes will not be accepted to this event.)

·  Participants will be subject to screening at Universal Studios™ prior to entry.

·  Once in park there are no “In and Out” privileges. Participants must remain in park for full event.

·  Senior Grad Nite tickets are non-refundable. No Exceptions.

·  The cost of the ticket includes transportation by charter buses and entry to Universal Studio™ Park from 5 P.M. to 2 A.M.

·  Ticket does not include food or drinks. (It is highly suggested to bring additional monies for expenses such as drink, food, souvenirs and arcade games)

·  Park will be closed to general public and open only to seniors from 8 P.M. until 2 A.M.

·  It is suggested by park to do the Universal Studio Tram Tour™ (which includes Fast and the Furious -The Ride™ and King Kong 360 3-D™) and any Show event when you first arrive. These events will close after 8 P.M. All other rides, including, but not limited to, Jurassic Park™, Harry Potter Wizarding World™, The Mummy™, The Walking Dead™, Despicable Me: Minion Mayhem™, Simpson’s Ride™ and Transformer’s Ride™ are open until 2 A.M. Also available after 8 P.M. are exclusive Senior Red Carpet events including DJ Dance Stations in Night Club Fashion and Park Restaurants.

·  TICKETS ARE NON-REFUNDABLE. If you are unable to attend or must cancel please contact VHHS PTSA. Unfortunately we cannot offer refund but we could try to facilitate ticket sales to another student before April 17, 2017. We are sorry for any inconvenience.

·  SPACE IS LIMITED! SIGN UP EARLY! Your non-refundable payment and completed forms reserve your space. The first 300 students to register are guaranteed a space. After that, a waitlist will be kept until we have enough students to fill another bus.

·  SPECIAL NEEDS: Please contact PTSA so we can help facilitate transportation and any other special needs for participants with disabilities.

·  Students will be assigned to a bus that they will both ride to and return from Universal Studios.

Universal Studios Senior Grad Bash Dress Code

Universal Studios Hollywood's Grad Bash is a once-a-year school event requiring enforcement of a dress code to ensure proper dress/attire for students and chaperones. All outfits must remain intact for the entire Universal Studios Hollywood's Grad Bash event and any Pre or Post Parties. We thank you in advance for your cooperation with our dress code.

GIRLS

·  Dresses, skirts, capris or Bermuda shorts, no shorter than mid-thigh

·  Casual pants (including jeans)

·  Casual blouses and tops with sleeves at least three inches wide

·  Comfortable shoes (including tennis shoes, dress sandals, boots and flip-flops)

·  Purses no larger than 8.5"XS.S" (size of a half sheet of paper)

·  Branded Logos (ex. Nike, American Eagle, Polo, etc.)

GUYS

·  Casual pants or shorts (including khakis and jeans), secured at waist

·  Shirts with sleeves (golf/polo, dress and T-shirts with sleeves), sport jackets and flip-flops

·  Girls may not wear tube tops, tube dresses, one shoulder shirts or any off the shoulder dresses or backless

clothing of any kind

PROHIBITED ITEMS

·  Weapons, knives, laser pointers, and chemical irritants of any kind

·  Over-the-counter medications, electronic cigarettes, tobacco or tobacco related products such as lighters,

matches, pipes, etc.; and alcohol or any controlled substance

·  Any containers with fluids of any kind; this includes water, any drinks and perfumes

·  Aerosol cans/bottles of any type

PLEASE NOTE: Some schools may enforce a stricter dress code.

Universal Studios Hollywood team members and Event Staff have the right to refuse admission to anyone whom they deem to be dressed inappropriately for the event and not in compliance with these guidelines, and will continue to enforce this dress code throughout the event. LAUSD POLICY FOR NON-LAUSD SPONSORED FIELD TRIPS: “An unauthorized field trip is a trip to a destination not inspected by the Districts OEHS and Risk Management Offices, not conducted during school hours, not sponsored or funded by the school, and not authorized by District. The District assumes no responsibility whatsoever in connection with extracurricular trips unrelated to the instructional programs or purely voluntarily school club rips, such as ski trips or other non-school related activities. Parents of students panning to participate in such extracurricular trips or school club trips must be informed in writing that District assumes no liability in connection with trip.”

We understand that all students will be transported by Brooks Transportation to the

We, the undersigned, acknowledge that participation in Senior Grad Nite is voluntary and we are voluntarily entering into this waiver and release liability with knowledge of the potential danger involved. The undersigned Parent/Guardian is allowing student participation in the activities with the knowledge that, as with any activity, physical injury as a possibility and herby agrees to accept any and all risk of injury. We hereby agree that this event is organized by VHHS PTSA volunteers and that we release any and all organizers and volunteers of this event from any claim that may arise out of student participation. We also agree that we will not make a claim against, sue or attach the property of the VHHS PTSA Senior Grad Nite Committee or any volunteer/member thereof. We also understand the event will observe a zero tolerance policy and that each student participant may be searched for illegal and inappropriate items.* We have carefully read this agreement and fully understand it’s contents.

*Grad Nite “Requirement-Code of Conduct-Dress Code” slip must be attached to this permission form to be considered complete. Without signatures on both forms VHHS PTSA has the right to refuse participation.

Ticket price covers the cost of the trip, transportation, and insurance. Refunds only if student does not qualify for trip per Verdugo Hills High School, VHHS PTSA and Universal Studios policy (See other form) prior to April 17, 2017. Returned checks will be subject to a fee and possible inability to attend trip. Amount due is based on the date the amount due is paid and submitted to PTSA

I have read, understood and will abide by the Verdugo Hills High School PTSA, Universal Studios™, and Brooks Transportation Requirements-Code of Conduct-Dress Code in order to attend the 2017 Senior Grad Nite Universal Studio Hollywood Bash™. Violation of any requirements or code of conduct rules can result in student’s inability to attend Senior Grad Nite. No refunds will be made for ticket cost if a student fails to adhere to all Requirements, Codes of Conduct, Dress Code.
